基于3D打印技术的术中导向装置引导髋臼周围通道螺钉治疗髋臼骨折的应用

摘要 目的观察基于3D打印技术的术中导向装置引导髋臼周围通道螺钉治疗髋臼骨折的应用价值。方法选取本院2019年3月-2021年1月收治的108例髋臼骨折患者,依据随机数字表法分为对照组、研究组,各54例;对照组采用传统手术治疗,研究组采用基于3D打印技术的术中导向装置引导髋臼周围通道螺钉治疗,术后均随访6个月;比较两组围手术期指标、骨折复位效果、髋关节功能及并发症发生情况。结果研究组手术耗时、总住院时间短于对照组,术中出血量、术中透视次数、术后引流量低于对照组,差异有统计学意义(P<0.05);研究组骨折复位效果优于对照组,差异有统计学意义(P<0.05);两组随访6个月髋关节功能评分均高于术前,且研究组髋关节功能评分高于对照组,差异有统计学意义(P<0.05);研究组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论髋臼骨折患者采用基于3D打印技术的术中导向装置引导髋臼周围通道螺钉治疗可缩短手术时间,减少术中出血量及透视次数,提高骨折复位效果及髋关节功能,可降低并发生发生率。 Objective To observe the application value of intraoperative guidance device based on 3D printing technology to guide periacetabular channel screws in the treatment of acetabular fractures.Methods The 108 patients with acetabular fracture from March 2019 to January 2021 were divided into control group and study group according to random number table method,with 54 patients in each group.The control groups were treated with traditional surgical treatment and the study group guided peripheral acetabular channel screws based on 3D printing technique.All patients were followed up for 6 months.Perioperative indexes,fracture reduction effect,hip function and complications were compared between the two groups.Results The operation time and total hospital stay of the study group were shorter than those of the control group,and the intraoperative blood loss,intraoperative fluoroscopy times and postoperative drainage volume were lower than those of the control group,the differences were statistically significant(P<0.05).The fracture reduction effect of the study group was better than that of the control group,and the difference was statistically significant(P<0.05).At 6 months follow-up,the hip function scores of both groups were higher than those before surgery,and the hip function scores of the study group were higher than those of the control group,the difference was statistically significant(P<0.05).The total complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In patients with acetabular fracture,intraoperative guidance device based on 3D printing technology to guide the periacetabular channel screw treatment can shorten the operation time,reduce the amount of bleeding and perspective times,improve the effect of fracture reposition and hip function,and reduce and occur.
