110例糖尿病足患者的营养不良发生状况及影响因素分析

摘要 目的探讨新疆建设兵团第一师医院糖尿病足病住院患者营养不良发生状况及其影响因素,采取营养干预,以促进糖尿病足患者恢复。方法收集2020年1月-2021年12月期间在新疆生产建设兵团第一师医院住院的110例糖尿病足患者的临床资料,采用微型营养评价法进行营养状况分类。结果营养异常组:占86.4%。其中,营养不良占44.5%(49/110),营养不良高危状态占41.9%(46/110)。营养良好组:占13.6(15/110)。营养异常组和营养良好2组在白细胞、血色素、白蛋白、总蛋白、糖化血红蛋白、Wagner分级的比较差异有统计意义(P<0.05)。多因素Logistic回归分析显示:血色素(OR=12.735,95%CI=1.435-113.033,95%CI P=0.022)、白蛋白(OR=9.886,95%CI=1.600-61.069)、ESR(OR=6.199,95%CI=1.198-32.083)、糖化血红蛋白(OR=5.785,95%CI=1.119-29.905)。结论对于糖尿病足病患者应早期对其进行营养宣教,进行干预,纠正贫血、低蛋白血症对糖尿病足病治疗及预后具有重要作用。 Objective To investigate the malnutrition status and its influencing factors of diabetes foot patients in the First Division Hospital of Xinjiang Construction Corps,and take nutritional intervention to promote the recovery of diabetes foot patients.Methods The clinical data of 110 patients with diabetes foot who were hospitalized in the First Division Hospital of Xinjiang Production and Construction Corps from January 2020 to December 2021 were collected,and the nutritional status was classified by micro-nutritional evaluation method.Results The malnutrition group accounted for 86.4%.Among them,malnutrition accounted for 44.5%(49/110),and the high-risk status of malnutrition accounted for 41.9%(46/110).Good nutrition group:13.6(15/110).There were statistically significant differences in leukocyte,hemoglobin,albumin,total protein,glycosylated hemoglobin and Wagner grading between the two groups(P<0.05).Multivariate logistic regression analysis showed that:hemoglobin(OR=12.735,95%CI=1.435-113.033,95%CI P=0.022),albumin(OR=9.886,95%CI=1.600-61.069),ESR(OR=6.199,95%CI=1.198-32.083),glycosylated hemoglobin(OR=5.785,95%CI=1.119-29.905).Conclusion For patients with diabetes foot disease,early nutrition education and intervention should be carried out to correct anemia and hypoproteinemia,which plays an important role in the treatment and prognosis of diabetes foot disease.
