肺癌合并肺栓塞患者的危险因素分析

Risk factors analysis in lung cancer patients combined with pulmonary emblism

目的通过分析肺癌合并肺栓塞患者及单纯肺癌患者的临床数据,探讨肺癌相关血栓形成的危险因素。方法选取新疆医科大学附属肿瘤医院2020年1月-2022年12月间住院的68例肺癌合并肺栓塞患者和136例单纯肺癌患者。通过SPSS 27.0对收集的临床数据进行统计学描述、单因素—差异因素分析、多因素—危险因素分析。结果单因素分析结果显示,年龄、下肢深静脉血栓形成、D-二聚体、病理类型(腺癌)及临床分期为两组差异因素(P值<0.05)。多因素分析结果显示,年龄、下肢深静脉血栓形成、D-二聚体、病理类型(腺癌)为肺癌患者相关肺栓塞形成的危险因素(P值<0.05),OR值分别为2.519、3.633、12.639、5.333。结论年龄>60岁、下肢深静脉血栓形成、D-二聚体>0.55 mg/L、病理类型为腺癌是肺癌患者肺栓塞发生的危险因素。

Objective To investigate the risk factors of lung cancer-related thrombosis by collecting and analyzing clinical data of patients with lung cancer combined with pulmonary embolism and patients with simple lung cancer.Methods A total of 68 patients with lung cancer combined with pulmonary embolism and 136 lung cancer patients who were hospitalized at the affiliated tumor hospital of Xinjiang medical university from January 2020 to December 2021 were selected.Statistical description,univariate differential factor analysis,and multivariate risk factor analysis were conducted to analyze the collected clinical data using SPSS 27.0.Results The results of univariate analysis showed that ages,lower limb deep venous thrombosis,D-dimer,pathological type(adenocarcinoma)and clinical stage were the difference factors between the two groups(P<0.05).The results of multivariate analysis showed that ages,lower limb deep venous thrombosis,D-dimer,lung adenocarcinoma were risk factors for pulmonary embolism in patients with lung cancer(P<0.05),with OR values of 2.519,3.633,12.639 and 5.333,respectively.Conclusion Ages over 60 years old,deep venous thrombosis,D-dimer>0.55mg/L and pathological types of adenocarcinoma are risk factors for pulmonary embolism in patients with lung cancer.
