腮腺良性肿瘤增强CT征象及其诊断价值

CT imaging signs of benign parotid tumors and their diagnostic value

目的分析腮腺多形性腺瘤(pleomorphic adenoma of parotid gland,PAPG)、沃辛瘤(warthin tumor,WT)、基底细胞腺瘤(basal cell adenoma,BAC)及嗜酸细胞腺瘤(parotid gland oncocytomas,PGO)的增强CT征象,提高对这几种疾病的诊断及鉴别诊断能力。方法回顾性分析经手术病理证实的12例PAPG、8例WT、18例BAC及6例PGO的增强CT征象,对病灶的位置、大小、形态、边缘、CT征象及强化方式等进行分析。结果PAPG、WT、BAC及PGO患者性别及病灶形态、囊变、坏死比较有差异(P<0.05),而年龄及病灶大小、数目、位置、钙化、边界比较无差异(P>0.05)。PAPG、WT及PGO病灶平扫期CT值比较有差异(P<0.05),而动脉期CT值、静脉期CT值、动脉期强化净增值及动-静脉期强化差值比较无差异(P>0.05)。结论PAPG、BAC、WT及PGO的CT征象上各有一定特征,综合分析有助于腮腺肿瘤的诊断及鉴别诊断。

Objective To analyze the CT features of pleomorphic adenoma of parotid gland(PAPG),warthin tumor(WT),basal cell adenoma(BAC)and parotid gland oncocytomas(PGO),and to improve the ability of diagnosis and differential diagnosis of these diseases.Methods CT features of 12 cases of PAPG,8 cases of WT,18 cases of BAC and 6 cases of PGO were analyzed retrospectively,all CT images were retrospectively evaluated with respect to location,size,morphology,margin,CT density and enhancement behavior.Results There were differences in gender,lesion morphology,cystic degeneration,and necrosis among PAPG,WT,BAC,and PGO(P<0.05),but no differences in ages,lesion size,number,location,calcification,and margin(P>0.05).The CT values of PAPG,WT and PGO lesions in plain scan were different(P<0.05),However,there was no difference in arterial phase CT value,venous phase CT value,arterial phase enhancement net enhancement,and arterial venous phase enhancement difference(P>0.05).Conclusion The CT images of PAPG,BAC,WT and PGO have certain characteristics,and comprehensive analysis is helpful for the diagnosis and differential diagnosis of parotid gland tumors.
