妊娠期糖尿病需胰岛素治疗与体质指数的相关性研究

Study about the correlation between need of insulin therapy and BMI in GDM

目的研究GDM需胰岛素治疗与妊娠前BMI的相关性。方法分析238例饮食运动控制血糖的GDM与162例胰岛素控制血糖的GDM的临床资料,比较两组妊娠前BMI与需胰岛素治疗之间的关系。进行Logistic回归分析GDM需胰岛素治疗的危险因素。结果(1)需胰岛素治疗组的年龄、产次、妊娠前体重和妊娠前BMI均高于饮食运动治疗组,两组比较有统计学意义。而合并妊娠期高血压需胰岛素治疗率高于饮食运动组,两组比较有统计学意义。需胰岛素治疗组的空腹、1 h、2 h血糖和糖化血红蛋白均高于饮食运动治疗组,两组比较有统计学意义。OGTT有3个异常指标需胰岛素治疗率高于饮食运动治疗组,两组比较有统计学意义;(2)年龄、妊娠前BMI、空腹血糖、2 h血糖、糖化血红蛋白和OGTT异常指标均是GDM需胰岛素治疗的危险因素。结论(1)饮食运动治疗组与需胰岛素治疗组相比,妊娠前BMI有统计学意义;(2)妊娠前BMI是GDM需胰岛素治疗的独立危险因素。

Objective To study the correlation between the need of insulin therapy and Pre-pregnancy BMI in GDM.Methods The clinical data of 238 cases of GDM with diet and exercise control and 162 cases of GDM with insulin control were analyzed,and the correlation between pre-BMI and insulin therapy was comparedin the two groups.Logistic regression was used to analyze the risk factors of need of insulin therapy in GDM.Results(1)Age,number of production,pre-weight and pre-BMI of the insulin therapy group were higher than those of the diet and exercise group,and there were statisticallysignificance between the two groups.The insulin therapy rate of patients with gestational hypertension was higher than that of the diet and exercise group,and there was statisticallysignificance between the two groups.The FBG,1hBG,2hBG and HbA1c in the insulin therapy group were higher than those in thediet and exercise group,and which were statisticallysignificance between the two groups.The rate of insulin therapyfor 3 abnormal indicators of OGTT was higher than that in the diet and exercise group,and there were statisticallysignificance between the two groups;(2)Age,pre-BMI,FBG,2hBG,HbA1c and the abnormal OGTT points wereallrisk factors for GDM need of insulin therapy.Conclusion(1)Pre-BMI in thediet and exercise group is higher than that in need of insulin therapy group;(2)pre-BMI is an independent risk factor for need of insulin therapy in GDM.
