短期胰岛素强化治疗初诊2型糖尿病的临床疗效

Analysis of the clinical effect of short-term intensive insulin therapy on newly diagnosed type 2 diabetes mellitus

目的观察使用短期胰岛素强化方案治疗的初诊2型糖尿病患者的临床效果。方法收集2021年8月-2022年12月在新疆医科大学第一附属医院内分泌科住院治疗的105例初诊的2型糖尿病患者的临床资料,对符合纳入标准的患者进行为期7 d-14 d的短期胰岛素强化治疗。观察治疗前后空腹血糖(FPG)、餐后2 h血糖(2 hPG)、空腹C肽(FC-P)及餐后2 hC肽(2 hC-P)变化,计算HOMA胰岛素抵抗指数(HOMA-IR)、HOMA-β功能指数(HOMA-β)。结果治疗后FPG、2 hPG、HOMA-IR均较治疗前降低,FC-P、2 hC-P、HOMA-β较治疗前有所升高,P<0.05,认为差异有统计学意义。结论对初诊2型糖尿病患者进行短期胰岛素强化治疗,可快速解除葡萄糖毒性,改善胰岛β细胞分泌功能,减轻机体胰岛素抵抗,有利于长期血糖控制,值得推广应用。

Objective To observe the clinical effect of short-term intensive insulin therapy in newly diagnosed patients with type 2diabetes.Methods Clinical data of 105 newly diagnosed patients with type 2 diabetes who were hospitalized in the Department of Endocrinology,the First Affiliated Hospital of Xinjiang Medical University from August 2021 to December 2022 were collected.Patients meeting the inclusion criteria were treated with short-term intensive insulin therapy for 7-14 days.The changes of fasting blood glucose(FPG),postprandial 2h blood glucose(2hPG),fasting C-peptide(FC-P)and postprandial 2hC peptide(2HC-P)were observed before and after treatment.Insulin resistance index(HOMA-IR)and HOMA-βfunction index(HOMA-β)were calculated.Results After treatment,FPG,2hPG and HOMA-IR were decreased compared with before treatment,while FC-P,2hC-P and HOMA-βwere increased compared with before treatment,P<0.05,the difference was considered statistically significant.Conclusion Short-term intensive insulin therapy for newly diagnosed type 2 diabetes patients can rapidly relieve glucose toxicity,improve the secretion function of pancreaticβcells,reduce insulin resistance,and contribute to long-term blood sugar control,and is worthy of popularization and application.
