不同药物预防下颌埋伏智齿拔除术后并发症的临床观察

Clinical Observation on the Prevention of Postoperative Complications of Mandibular Implanted Wisdom Teeth Extraction with Different Drugs

目的观察不同药物预防埋伏阻生智齿拔除术后并发症的发生情况。方法选取2020年3月-2021年10月在口腔医院行下颌埋伏阻生智齿拔除术的患者400例,随机分为实验组和对照组,其中CGF组(A1)100例、胶原蛋白组(A2)100例、明胶海绵组(A3)100例及对照组(A4)100例。术后对拔牙创进行常规处理,分别填入CGF、胶原蛋白海绵、明胶海绵后缝合及直接缝合止血。观察拔牙术后拔牙创口出血、术后第1 d及第3 d拔牙创疼痛、面部组织肿胀、拔牙创口感染及干槽症等相关并发症的发生情况。结果实验组拔牙术后的出血率、拔牙创疼痛率、感染率及肿胀率低于对照组(P<0.05);实验组拔牙术后干槽症的发生率与对照组无明显差异(P>0.05);CGF组在拔牙术后的出血率、肿胀率、感染率低于明胶海绵组(P<0.05);术后疼痛率CGF组低于胶原蛋白组低于明胶海绵组(P<0.05)。结论CGF、胶原蛋白海绵及明胶海绵均可以有效降低下颌第三磨牙拔牙术后并发症的发生,其中CGF疗效最佳。

Objective To evaluates the application of concentrated different drugs in preventing the complications after mandibular low impacted third molar extraction.Methods 400 patients with mandibular low impacted teeth extraction in our hospital during March 2020 and October 2021 were randomly divided into experimental group and blank group,such as 100 cases were in the CGF group(A1),100 cases in the collagen group(A2),100 cases in the gelatin sponge group(A3)and 100 cases in the control group(A4).Filling CGF,collagen sponge,and gelatin sponge respectively,followed by suture and direct suture to stop bleeding after routine management of postoperative tooth extraction wounds.Thebleeding,pain,infection,swellingand dry socket of the wound after operation were observed.Results the bleeding rate,pain rate,infection rate and swelling rate of tooth extraction in the experimental group were lower than those in the blank group(P<0.05);There was no significant difference in the incidence of dry socket between the two groups(P>0.05);the bleeding rate and infection rate and swelling rate of tooth extraction in the CGF group were lower than in the gelatinsponge group(P<0.05)and the postoperative pain rate in the CGF group was lower than thatin the collagen group and lower than that in the gelatin sponge group(P<0.05).Conclusion CGF,Collagen sponge and gelatin sponge can effectively prevent the occurrence of complications after tooth extraction among which CGF has the best effect.
