利用远程及纹理分析提高基层高原肺水肿的诊断能力

摘要 目的 利用网络平台、远程传输和纹理分析的方法提高喀什偏远高原地区对于高原肺水肿的诊断和鉴别能力。方法 回顾性分析经喀什地区塔什库尔干县远程传输并进行纹理分析的病例56例,其中临床确诊的21例高原肺水肿及35例肺部炎症。所有患者均行CT扫描,进行组间比较,评估远程及纹理分析提升的基层鉴别能力。结果 经远程会诊前后两者鉴别诊断能力差异存在统计学意义(P <0.05);将高原肺水肿与双肺炎症CT图像进行纹理分割,得到10个纹理参数两者间差异存在统计学意义(P <0.05)。结论 通过远程会诊和CT图像的纹理分析参数可以明显提升基层偏远地区对于高原肺水肿的诊断并采取积极治疗。 Objective The network platform,remote transmission and texture analysis were used to improve the ability of diagnosis and differentiation of high altitude pulmonary edema in the remote plateau area of Kashgar.Methods A retrospective analysis was performed on 56 cases transmitted by remote transmission and texture analysis in Tashkurgan county,Kashi region.Among them,21 cases of high altitude pulmonary edema and 35 cases of pulmonary inflammation were clinically confirmed.CT scanning was performed in all patients for comparison between groups to evaluate the discriminationabilityof remote and texture analysisat grass-root area.Results There was significant difference in differential diagnosis ability between the two groups before and after remote consultation(P<0.05).CT images of high altitude pulmonary edema and double pneumonia were segmented to obtain 10 texture parameters,and there was statistical significance in 10 texture parameters between them(P<0.05).Conclusion Remote consultation and texture analysis parameters of CT images can significantly improve the diagnosis and treatment of high altitude pulmonary edema in remote areas.
