经外侧扩展入路接骨板内固定治疗Sanders Ⅲ型跟骨关节内骨折病人对功能恢复的影响

摘要 目的 分析Sanders Ⅲ型跟骨关节内骨折病人经外侧扩展入路接骨板内固定治疗对功能恢复产生的影响。方法2020年1月-2020年7月从本院选取Sanders Ⅲ型跟骨关节内骨折患者50例作为研究对象,随机分为对照组(n=25,行传统外侧L型入路接骨板内固定治疗)和观察组(n=25,行经外侧扩展入路接骨板内固定治疗),比较两组患者手术相关情况、足踝功能评分改善情况以及术后并发症发生情况。结果 术前等待时间、手术时间组间以及术中出血量无显著差异(P> 0.05),观察组住院时间显著短于对照组(P <0.05)。两组足踝功能评分术前无显著差异(P> 0.05),手术后评分均升高,且观察组显著高于对照组(P <0.05)。腓肠神经损伤、拐角坏死去除钛板、深部感染以及浅表感染等术后并发症发生率观察组低于对照组,差异显著(P <0.05)。结论 经外侧扩展入路接骨板内固定治疗Sanders Ⅲ型跟骨关节内骨折患者的临床效果突出,缩短了患者的住院时间,改善了足踝功能,且安全性较高,促进了患者病情的康复。 Objective To analyze the effect of internal fixation on the functional recovery of patients with Sanders type Ⅲ calcaneal intra-articular fractures through lateral extension approach.Methods From January 2020 to July 2020,50 patients with Sanders type Ⅲ calcaneal intra-articular fracture were selected from our hospital as the research subjects,and they were randomly divided into control group(n=25,treated with traditional lateral L-shaped approach bone plate internal fixation)and Observation group(n=25,underwent lateral extension approach bone plate internal fixation),and the operation-related conditions,improvement of ankle function scores and postoperative complications were compared between the two groups.Results There was no significant difference in preoperative waiting time,operation time and intraoperative blood loss(P>0.05),and the hospital stay in the observation group was significantly shorter than that in the control group(P<0.05).There was no significant difference in the scores of foot and ankle function between the two groups before operation(P>0.05),and the scores increased after operation,and the observation group was significantly higher than the control group(P<0.05).The incidence of postoperative complications such as sural nerve injury,removal of titanium plate from corner necrosis,deep infection and superficial infection in the observation group was lower than that in the control group,with a significant difference(P<0.05).Conclusion The clinical effect of internal fixation via lateral extension approach in the treatment of Sanders type III calcaneal intra-articular fractures is outstanding,the hospitalization time of the patients is shortened,the function of the foot and ankle is improved,and the safety is high,which promotes the improvement of the patient's conditionrecovery.
