麦角新碱预防选择性剖宫产产后出血的效果

Effect of Ergonovine in preventing postpartum hemorrhage after selective cesarean section

目的分析麦角新碱对选择性剖宫产产妇产后出血的预防效果。方法选取2020年6月-2021年9月乌鲁木齐市妇幼保健院产科收治的98例剖宫产产妇为研究对象,根据随机数字表法分为对照组(49例)和研究组(49例)。对照组于产妇刚分娩时为产妇肌注缩宫素注射液治疗,研究组在对照组的基础上另肌注马来酸麦角新碱治疗。两组用药后均观察24 h。比较两组分娩2h、2h-24 h、24 h的出血量、产后24 h的血红蛋白含量、生命体征、凝血功能、安全性。结果研究组分娩2 h-24 h出血量、分娩24 h出血量少于对照组(P<0.05);用药24 h后,研究组血红蛋白含量高于对照组(P<0.05)。与分娩前比较,分娩2h后,两组血浆纤维蛋白原(FIB)、D-二聚体(D-D)水平降低,研究组高于对照组(P<0.05),两组血浆凝血酶时间(TT)、凝血酶原时间(PT)延长,研究组短于对照组(P<0.05)。用药后,研究组与对照组不良反应发生率(14.29%vs 8.16%)比较,差异无统计学意义(P<0.05)。结论麦角新碱可调节剖宫产产妇凝血因子水平,缩短凝血时间,进而减少产妇产后出血量,同时,还具有良好的安全性。

Objective To analyze the preventive effect of Ergonovine on postpartum hemorrhage in women undergoing selective cesarean section.Methods 98 pregnant women undergoing cesarean sectionwho wereadmitted in Urumqi maternal and child health hospital from June 2020 to September 2021 were selected as the research objects.According to the random number table method,they were divided into control group(49 cases)and study group(49 cases).The control group were treated by intramuscular injection of oxytocin injection at childbirth,and the study group were treated by intramuscular injection of Ergometrine maleate on the basis of the control group.Both groups were observed for 24 hours.Blood loss,hemoglobin content,vital signs,coagulation function and safety were compared between the two groups.Results The blood loss at 2 to 24 h after delivery of the study group was less than that of the control group(P<0.05).After 24 h of treatment,the hemoglobin content of the study group was higher than that of the control group(P<0.05).Compared with before delivery,2 h after delivery,the levels of plasma fibrinogen(FIB)and D-dimer(DD)in the two groups were both decreased,butthe levels of the study group was higher than those of the control group(P<0.05).The plasma thrombin time(TT)and prothrombin time(PT)of the two groups were bothprolonged,but thatof the study group was shorter than that of the control group(P<0.05).After medication,there was no statistically significant difference in the incidence of adverse reactions between the study group and the control group(14.29%vs 8.16%)(P<0.05).Conclusion Ergonovine can adjust the level of coagulation factors in womenunderwentthe cesarean sectiontherapy,shorten the coagulation time,and then reduce the amount of postpartum hemorrhage of the parturients.At the same time,it is safe in clinic.
