单核/淋巴细胞比率与干燥综合征相关性分析

Correlation analysis between monocyte/lymphocyte ratio and primary Sjogren's syndrome

目的探讨单核/淋巴细胞比率(MLR)与原发性干燥综合征(primary Sjogren’s syndrome,pSS)及其活动性指标的相关性。方法采用回顾性研究方法,纳入2018年10月-2022年12月期间在新疆维吾尔自治区中医院风湿科门诊或住院当日检测血常规的pSS患者114例为pSS组,同期纳入在新疆维吾尔自治区中医院体检中心有血常规记录的健康体验者102例为健康对照组,健康对照组纳入时与pSS组在性别、年龄上匹配。首先比较pSS组与健康体检组MLR的差别并进行统计分析;其次根据C反应蛋白(CRP)、血沉(ESR)、疾病活动度(pSSDAI)等活动性指标的正常值参考范围将pSS分为异常组与正常组两个亚组,比较这两个亚组MLR的差别并进行统计学分析。以P<0.05为差异有统计学意义。结果pSS组与健康对照组两组具有可比性,两组单核细胞、淋巴细胞没有统计学差异(P>0.05),两组MLR有显著统计学差异(P<0.01)。pSS亚组比较发现,CRP异常组MLR高于CRP正常组(P<0.05),ESR、SSDAI异常组与正常组MLR无差别(P>0.05),提示MLR在急性炎症反应中增高。结论MLR在pSS患者中明显异常,一定程度上可作为病情急性活动的指标。

Objective To explore the correlation between monocyte/lymphocyte ratio(MLR)and primary Sjogren’s syndrome(pSS)and its activity index.Methods A retrospective study was used to include 114 patients with pSS who had blood routine tests at the outpatient department of the Rheumatology Department of the Xinjiang Hospital of Traditional Chinese Medicine or on the day of hospitalization as the pSS group during the period from October 2018 to December 2022,and 102 health cases who had blood routine records at the physical examination center of the Xinjiang Hospital of Traditional Chinese Medicine as the healthy control group.The healthy control group was matched with the pSS group in terms of gender and age when it was included.Firstly,the difference of MLR between pSS group and health examination group was compared and analyzed statistically;Secondly,according to the reference range of normal values of C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),disease activity index(pSSDAI),pSS was divided into two subgroups:abnormal group and normal group.The differences of MLR between the two subgroups were compared and statistically analyzed.P<0.05 was considered statistically significant.Results The pSS group and the healthy control group were comparable.There was no significant difference in monocyte count and lymphocyte count between the two groups(P>0.05),and there was significant difference in MLR between the two groups(P<0.01).The comparison of pSS subgroups showed that the MLR of the abnormal CRP group was higher than that of the normal CRP group(P<0.05),and the MLR of the abnormal ESR and SSDAI group was no different from that of the normal group(P>0.05),suggesting that the MLR was increased in the acute inflammatory reaction.Conclusion MLR is obviously abnormal in patients with pSS and to some extent maybe used as an indicator of acute inflammatory of the disease.
