多维健康教育在门诊妇科无痛手术中的应用效果

摘要 目的探讨多维健康教育在门诊妇科无痛手术中的应用效果。方法选择2019年1月-2020年10月拟在本院行无痛手术治疗患者90例,根据随机数字表法分为两组,每组45例。对照组采用常规护理,在此基础上,干预组加用多维健康教育。比较两组患者心理韧性(心理弹性量表(CD-RISC)评分)、术前宣教依从性、手术延期与麻醉方式改变原因,并使用创伤后应激障碍自评量表(PCL-C)测定两组应激状态。结果护理前两组CD-RISC评分比较,差异无统计学意义(P>0.05);护理后,干预组CD-RISC各维度评分均较对照组高,差异有统计学意义(P<0.05);干预组术前宣教依从性高于对照组,无家属陪同、术前检查不完善、未禁食禁饮比例低于对照组,差异有统计学意义(P<0.05);护理前两组PCL-C评分比较,差异无统计学意义(P>0.05);护理后,干预组PCL-C评分较对照组低,差异有统计学意义(P<0.05)。结论门诊妇科无痛手术中使用多维健康教育干预可提高患者术前宣教依从性,降低麻醉方式改变、延期手术发生率,能有效减轻患者应激状态、提高心理韧性。 Objective To investigate the effect of multidimensional health education in painless gynecological surgery.Methods A total of 90 patients who planned to receive painless surgical treatment in our hospital from January 2019 to October 2020 were selected and divided into two groups according to random number table method,with 45 patients in each group.The control group was treated with routine nursing,and the intervention group wasgiven multidimensional health educationon this basis of control group.The psychological toughness(psychological resilience scale(CD-RISC)score),compliance of preoperative education,operation delay and reasons for changing anesthesia methodsbetween two groups were compared.Posttraumatic stress disorder self-rating scale(PCL-C)was used to measure the stress state of the two groups.Results Before nursing,there was no significant difference in CD-RISC score between the two groups(P>0.05).After nursing,CD-RISC score in the intervention group was higher than that in the control group,with statistically significant difference(P<0.05).The preoperative education compliance of the intervention group was higher than that of the control group,and occurrence rate of some situations including no accompany of family members,imperfect preoperative examination,non-fastingin the intervention group was lower than those of the control group and the differences were statistically significant(P<0.05).There were no significant differences in PCL-C scores between the two groups before nursing(P>0.05).After nursing,the PCL-C score of the intervention group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Multidimensional health education intervention in gynecological painless surgery in outpatient service can improve preoperative education compliance of patients,reduce the change of anesthesia mode and the incidence of delayed surgery,effectively relieve patients'stressand improve their mental toughness.
