显微镜下通道技术治疗椎间盘突出与传统开放手术短期疗效对比分析

Comparative analysis about short-term efficacy of microscopical channel technique and traditional open surgeryin the treatment of intervertebral disc herniation

目的探究对椎间盘突出患者分别施行传统开放手术、显微镜下通道技术治疗的效果。方法采用随机数字表法将2018年8月-2020年6月本院72例椎间盘突出患者分为对照组(36例,施行传统开放手术治疗)、观察组(36例,施行显微镜下通道技术治疗)。随访半年,对比两组手术情况、治疗优良率、疼痛评分、腰椎功能评分。结果(1)观察组手术用时、术中出血量、术后引流量、术后卧床时间均较对照组更少,P<0.05;(2)观察组治疗优良率(83.33%)和对照组(80.56%)对比无显著差异,P>0.05;(3)观察组术后1个月、3个月疼痛评分均较对照组更低,P<0.05,但两组术后半年疼痛评分对比无显著差异,P>0.05;(4)观察组术后1个月、3个月腰椎功能评分均较对照组更低,P<0.05,但两组术后半年腰椎功能评分对比无显著差异,P>0.05。结论对椎间盘突出患者施行显微镜下通道技术治疗可达到和传统开放手术较相似的效果,但显微镜下通道技术创伤更小,术后恢复更快,且能更快改善腰椎功能。

Objective To explore the effect of traditional open surgery and microscopic access technology in treating patients with intervertebral disc herniation.Methods 72 patients with intervertebral disc herniation in our hospital from August 2018 to June 2020were randomly divided into control groupand observation group.There were 36 patients in the control group who were treated with traditional open surgery and 36 patients in the observation group who were treated with channel technology under microscope.After followed up for half a year,operation condition,excellent and good rate of treatment,pain score,lumbar function score were compared between two groups.Results(1)The operation time,intraoperative blood loss,postoperative drainage,and postoperative bedtime in the observation group were less than those in the control group(P<0.05).(2)There was no significant difference about the excellent and good rate of treatment in the observation group(83.33%)and the control group(80.56%)(P>0.05).(3)The pain score of the observation group were lower than that of the control group at one month and three months after the operation(P<0.05),but there was no significant difference in the pain scores between the two groups at six months after the operation(P>0.05).(4)Lumbar function scores of the observation group were lower than those of the control group at one month and three months after the operation(P<0.05),but there was no significant difference in the lumbar function scores between the two groups at half a year after the operation(P>0.05).Conclusion The treatment of patients with intervertebral disc herniation can achieve similar results with traditional open surgery.but it has less trauma,faster postoperative recovery,and faster improvement in lumbar spine function.
