显微镜精索静脉结扎术与腹腔镜精索静脉曲张高位结扎术疗效的对比

Comparison between microscopical varicocele ligation and laparoscopic varicocele high ligation

目的研究显微镜精索静脉结扎术与腹腔镜精索静脉曲张高位结扎术疗效。方法选择本院2020年08月-2022年06月诊断为精索静脉曲张并行手术治疗的患者,其中显微镜组50例,腹腔镜组50例,对比两组患者术前术后精子质量、术后并发症及3个月的复发率。结果在基线资料无统计学差异的情况下,两组患者手术均取得成功,在手术时间上,显微镜组患者较腹腔镜组患者时间较长(P<0.05),但显微镜组患者较腹腔镜组患者时间较短(P<0.05)。在术后并发症方面,术后疼痛缓解及复发方面,两组无明显差异(P>0.05),但在术后阴囊水肿的发生率中,腹腔镜组明显高于显微镜组(P<0.05)。两组患者术后及术后3个月精子密度与前向运动精子百分率均有明显改善,且两组患者均恢复至同一水平。结论显微镜精索静脉结扎术治疗精索静脉曲张疗效确切,为治疗精索静脉曲张提供了一种可选择的治疗方法。

Objective To study the efficacy of microscopical varicocele ligation and laparoscopic varicocele high ligation.Methods Patients with varicocele diagnosed and treated by surgery in our hospital from August 2020 to June 2022 were selected,including 50 cases in the microscope group and 50 cases in the laparoscopy group.The preoperative and postoperative sperm quality,postoperative complications and 3-month recurrence rate of the two groups were compared.Results In the absence of statistical differences in baseline data,the operation was successful in both groups.In terms of operation time,the patients in the microscope group were longer than those in the laparoscope group(P<0.05),but the patients in the microscope group were shorter than those in the laparoscope group(P<0.05).In terms of postoperative complications,postoperative pain relief and recurrence,there was no significant difference between the two groups(P>0.05),but in the incidence of scrotal edema after surgery,the laparoscopic group was significantly higher than the microscope group(P<0.05).The sperm density and the percentage of forward moving sperm in the two groups were significantly improved after operation and 3 months after operation,and the patients in both groups recovered to the same level.Conclusion The microscopical varicocele ligation has a definite effect on the treatment of varicocele and provides an alternative treatment for varicocele.
