以医联体为载体的分级诊疗服务在社区慢性病管理中的应用效果观察

Observation on the application effect of hierarchical diagnosis and treatment service based on medical consortium in community chronic disease management

目的分析以医联体为载体的分级诊疗服务在社区慢性病管理中的应用效果。方法选定本院社区2017年6月-2019年6月接诊的102例慢性病患者,以双盲随机抽样法分组(每组病例数总计51例),对照组采纳常规社区诊疗服务,实验组采纳以医联体为载体的分级诊疗服务,对比两组AHSMSRS评分、血脂、血压、血糖达标率、SF-36评分。结果实验组诊疗服务后AHSMSRS评分、SF-36评分均高于对照组,实验组血脂(94.74%)、血压(93.75%)、血糖达标率(93.75%)均高于对照组(65.00%、55.56%、61.54%),P<0.05(差异均具有统计学意义)。结论以医联体为载体的分级诊疗服务可提高社区慢性病患者自我管理能力、血脂、血压、血糖达标率,改善生活质量,值得借鉴。

Objective To analyze the application effect of graded diagnosis and treatment services based on medical consortium in the management of chronic diseases in the community.Methods The 102 patients with chronic diseases admitted from June 2017 to June2019 in the community of this hospital were selected and grouped by double-blind random sampling(the number of cases in each group was 51).The control group adopted conventional community diagnosis and treatment services.The experimental group adopted hierarchical diagnosis and treatment service with medical association as carrier,and compared AHSMSRS score,blood lipid,blood pressure,blood glucose compliance rate and SF-36 score of the two groups.Results The AHSMSRS and SF-36 scores of the experimental group after nursing were higher than those of the control group.The blood lipid(94.74%),blood pressure(93.75%),and blood glucose compliance rate(93.75%)of the experimental group were higher than those of the control group(65.00%,55.56%,61.54%),P<0.05(the differences are all statistically significant).C onclusion The hierarchical diagnosis and treatment service based on the medical consortium can improve the self-management ability,blood lipid,blood pressure,and blood sugar compliance rate of patients with chronic diseases in the community,and improve the quality of life,which is worth learning.
