脊柱结核CT与MRI的影像诊断对提升诊断准确性的效果比较

Effect comparision of CT and MRI imaging diagnosis of spinal tuberculosis on improving the accuracy of diagnosis

目的探讨脊柱结核CT与MRI的影像诊断对提升诊断准确性的效果。方法以2019年1月至2020年1月在本院治疗的50例脊柱结核患者为例展开研究,对其分别进行CT检查和MRI检查,比较其诊断准确性、检查时间以及检查满意率。结果两种诊断方式在椎旁肿块或脓肿检查方面以及椎体、椎间盘破坏检查方面的诊断准确性比较,差异无统计学意义(P>0.05);CT在死骨钙化检查方面的诊断准确性要显著高于MRI诊断,但是在韧带下扩散检查方面、脊膜强化检查方面以及硬膜外或锥孔侵犯检查方面的诊断准确性显著低于MRI诊断(P<0.05)。CT的检出时间以及检查总用时和MRI比较,均显著更短,但是其诊断总满意率显著低于MR(P<0.05)。结论CT以及MRI在脊柱结核检查诊断中各具优势,前者对死骨钙化的诊断准确性更高,检查耗时短,后者对韧带下扩散、脊膜强化及硬膜外或锥孔侵犯的诊断准确性更高,检查满意度高,临床可以根据实际情况必要时进行联合诊断,以提高诊断准确性以及满意度。

Objective To investigate the effect of CT and MRI imaging diagnosis of spinal tuberculosis on improving the accuracy of diagnosis.Methods A study was conducted on 50 cases of spinal tuberculosis patients treated in our hospital from January 2019 to January 2020.CT examinations and MRI examinations were performed on them,and their diagnostic accuracy,examination time and examination satisfaction rate were compared.Results The comparison of the diagnostic accuracy of the two diagnostic methods in the inspection of paravertebral masses or abscesses and the inspection of vertebral body and intervertebral disc destruction was not statistically significant(P>0.05);the diagnostic accuracy of CT in the inspection of calcification of the dead bone was significantly higher than MRI diagnosis,but the diagnostic accuracy of subligament diffusion examination,spinal membrane enhancement examination,and epidural or cone invasion examination was significantly lower than MRI diagnosis(P<0.05).Compared with MRI,the detection time of CT and the total time of examination were significantly shorter,but the overall satisfaction rate of diagnosis was significantly lower than that of MRI(P<0.05).Conclusion CT and MRI have their own advantages in the diagnosis of spinal tuberculosis.The former is more accurate in the diagnosis of sequestered bone calcification and the examination is shorter.The latter is more effective in subligament spread,spinal membrane enhancement,and epidural or cone invasion,the diagnosis accuracy is higher,and the inspection satisfaction is high.The clinic can perform joint diagnosis according to the actual situation to improve the diagnosis accuracy and satisfaction.
