摘要
目的分析血浆1,3-β-D葡聚糖测定(G试验)在肺部真菌感染患者的诊断意义。方法从2019年3月至2020年8月本院呼吸科住院的患者中,随机抽取肺真菌病患者80例为A组、无肺部真菌感染患者80例组成B组,以及自愿参与研究的健康志愿者80例组成的C组,三组均先后接受血浆1,3-β-D葡聚糖测定和传统的痰涂片法检查。随后对三组的血浆1,3-β-D葡聚糖测定结果及痰涂片结果进行统计学分析,探讨血浆1,3-β-D葡聚糖测定法的临床诊断意义。结果A组、B组和C组的1,3-β-D葡聚糖值分别为:52.03±50.98;6.71±5.49;6.63±4.27 pg/ml。对A、B、C三组结果采用统计学分析,结果A组、B组、C三组的葡聚糖含量存在统计学差异(P<0.05),B、C两组的葡聚糖含量无统计学差别(P>0.05)。可见G试验的肺部真菌感染检测结果具有较高的准确度。A组1,3-β-D葡聚糖测定结果阳性者78人,检出率为97.5%,痰涂片法检测结果阳性者69人,检出率为86.25%。G试验的阳性率要高于痰涂片培养,两者阳性率之间的差异具有统计学意义(P<0.05)。提示G试验的灵敏度要优于痰涂片培养法。结论G试验再检测肺部真菌感染患者时,阳性检出率和检出准确度都较高,并且出结果更快,值得在肺部真菌感染检查诊断中推广。
Objective To analyze the diagnostic significance of plasma 1,3-β-D-glucan(g test)in patients with pulmonary fungal infection.Methods From March 2019 to August 2020,80 patients with pulmonary mycosis,80 patients without pulmonary fungal infection,and 80 healthy volunteers were divided into group A,group B and group C.Then the plasma 1,3-β-D glucan test results and sputum smear results of the three groups were statistically analyzed to explore the clinical diagnostic significance of plasma 1,3-β-D glucan determination.Results The amounts of 1,3-β-D glucan in group A,B and C were 52.03±50.98,6.71±5.49 and 6.63±4.27 pg/ml,respectively.The results of group A,B and C were statistically analyzed.There was a significant difference in the content of glucan in group A,group B and group C(P<0.05),but there was no significant difference between group B and group C(P>0.05).It can be seen that the detection results of G test for pulmonary fungal infection have high accuracy.In group A,78 cases were positive for 1,3-β-D glucan,the detection rate was 97.5%.69 cases were positive by sputum smear method,the detection rate was 86.25%.The positive rate of G test was higher than that of sputum smear culture(P<0.05).It was suggested that the sensitivity of G test is better than that of sputum smear culture.Conclusion when G test is used to detect patients with pulmonary fungal infection,the positive detection rate and detection accuracy are higher,and the results are faster,which is worthy of promotion in the diagnosis of pulmonary fungal infection.
