新疆某三甲医院ICU护理人员对ICU谵妄认知水平及实践的现况调查

Investigation about cognition level and practice of ICU nurses to ICU delirium in a grade A hospital in Xinjiang

目的了解新疆某三甲医院ICU护士对ICU谵妄的认知水平及临床实践的现况,分析现况的影响因素,为ICU管理者今后制订相应培训方案提供理论依据。方法采用方便抽样法,在德尔菲法的基础上,将自行设计《ICU护理人员对ICU谵妄的认知与临床实践现状调查》问卷对新疆某三甲医院的278名ICU护理人员进行调查。结果(1)ICU护理人员对ICU谵妄的认知现状:ICU谵妄的分型、作用机制、评估工具、并发症、预后等方面的认知程度较低,完全熟悉和比较熟悉的在2.9-24.9%之间;熟悉和完全不熟悉的在3.2-80.0%之间;(2)ICU护理人员对ICU谵妄的临床实践情况:目前使用的ICU谵妄评估工具多为重症监护室意识模糊评估法、ICU患者谵妄筛查表和格拉斯哥昏迷量表;(3)ICU护理人员对ICU谵妄的基本认知与临床实践情况的得分分别为(25.21±5.32)、(18.56±3.03)分,二者呈正相关(r=0.51,P<0.05)。结论新疆某三甲医院的ICU护理人员对ICU谵妄的认知水平和临床实践的能力存在一定的问题,但是对ICU谵妄的管理态度较积极。因此,ICU护理管理者今后应加强对ICU谵妄相关知识的培训力度,开展多种形式,鼓励ICU护理人员积极加入ICU谵妄的管理中,从而提高ICU护理人员对ICU谵妄的管理水平和满足ICU患者科学管理的需求。

Objective To understand the cognitive level of ICU nurses about delirium and the current situation of clinical practice in a third-grade a hospital in Xinjiang,and to analyze the influencing factors of the current situation,and provide theoretical basis for ICU managers to make corresponding training programs in the future.Methods Based on the Delphi method,278 ICU nurses in a grade A hospital in Xinjiang were investigated with the questionnaire named"Investigation on the Cognition and Clinical Practice of ICU Nurses in Delirium".Results(1)The status quo of ICU nurses’cognition of ICU delirium:the cognition degree of ICU delirium classification,mechanism of action,assessment tools,complications,prognosis and other aspects was relatively low,and those who were completely familiar with and relatively familiar with ICU delirium ranged from 2.9 to 24.9%.Nurses who were familiar and completely unfamiliar ICU delirium were between 3.2-80.0%.(2)Clinical practice of ICU nursing staff on delirium in ICU:Currently,most of the tools used to evaluate delirium in ICU were the consciousness ambiguity assessment method,the delirium screening table for ICU patients and the Glasgow Coma Scale.(3)The scores of basic cognition and clinical practice of ICU nurses on delirium were(25.21±5.32)and(18.56±3.03),respectively,which were positively correlated(r=0.51,P<0.05).Among them,professional title and working years were the main factors that affect ICU nursing staff’s cognition of ICU delirium(F=27.31,39.66,P<0.05).The main factors affecting the clinical practice of ICU nurses’delirium are working years,professional title,and ICU category(F=12.82,8.32,7.26;T=4.65,P<0.05).Conclusion There are some problems in the cognitive level and clinical practice ability of ICU nurses in a grade A hospital in Xinjiang province,but they have a positive attitude towards management of ICU delirium.Therefore,ICU nursing managers should strengthen the training of relevant knowledge of ICU delirium in the future,carry out various forms,and encourage ICU nursing staff to actively participate in the management of ICU delirium,so as to improve the management level of ICU nursing staff to ICU delirium and meet the needs of scientific management of ICU patients.
