微波静脉腔内凝固联合泡沫硬化剂加大隐静脉高位结扎治疗下肢静脉曲张患者的临床疗效

Clinical study on endovenous microwave ablation combined with foam sclerotherapy and high ligation of great saphenous vein for varicose veins of lower extremities

目的探讨微波静脉腔内凝固联合泡沫硬化剂加大隐静脉高位结扎治疗下肢静脉曲张患者的临床疗效。方法选取北京市大兴区人民医院2018年2月至2019年8月收治的242例(377条肢体),根据治疗方式不同分为联合治疗组(n=151,234条肢体)和传统组(n=91,143条肢体)。观察并比较两组患者手术指标(手术时间及术中出血情况)及住院时间;记录两组患者治疗前及治疗后6个月静脉临床严重程度评分(VCSS)和阿伯丁静脉曲张问卷(AVVQ)评分的变化,评价两组患者临床疗效。结果联合治疗组患者手术及住院时间均短于传统组患者,术中失血量少与传统组患者,差异有统计学意义(P<0.05)。治疗后6个月,两组组内患者VCSS与AVVQ评分均较治疗前下降,差异有统计学意义(P<0.05)。结论微波静脉腔内凝固联合泡沫硬化剂加大隐静脉高位结扎治疗下肢静脉曲张临床疗效显著,术后恢复好,值得临床推荐。

Objective To investigate the clinical effect of endovenous microwave ablation combined with foam sclerotherapy and high ligation of great saphenous vein for varicose veins of lower extremities.Method 242 cases(377 limbs)were selected from Daxing District People’s Hospital of Beijing from February 2018 to August 2019.According to different treatment methods,they were divided into combined treatment group(n=151,234 limbs)and traditional group(n=91,143 limbs).To observe and compare the operation indexes(operation time and intraoperative bleeding)and hospitalization time of the two groups;to record the changes of the venous clinical severity score(VCSS)and the aberdeen varicose vein questionnaire(AVVQ)score before and after treatment in the two groups,and to evaluate the clinical efficacy of the two groups.Result The time of operation and hospitalization in the combined treatment group was shorter than that in the traditional group,and the blood loss in the operation was less than that in the traditional group,the difference was statistically significant(P<0.05).After 6 months of treatment,the scores of VCSS and AVVQ in the two groups decreased significantly(P<0.05).Conclusion Microwave coagulation combined with foam sclerotherapy plus high saphenous vein ligation is effective in the treatment of varicose veins of lower extremities.
