摘要
目的研究Angiojet机械吸栓联合置管溶栓(CDT)治疗下肢深静脉血栓形成(LEDVT)的疗效。方法选取佳木斯大学附属第一医院2017年07月至2019年04月收治的LEDVT患者60例。根据溶栓方式不同分为对照组和观察组,各30例。比较两组患者治疗总有效率和治疗情况。术后6个月评估患者静脉通畅情况和深静脉血栓形成后综合征(PTS)发生情况。结果观察组患者治疗总有效率为100.00%高于对照组患者73.33%;尿激酶用量低于对照组患者,溶栓和住院时间均短于对照组患者,差异有统计学意义(P<0.05)。术后6个月,观察组患者静脉通畅率为86.67%高于对照组患者33.33%,差异有统计学意义(P<0.05)。观察组患者PTS率为3.33%低于对照组患者13.33%,差异无统计学意义。结论LEDVT患者采取Angiojet机械吸栓联合CDT治疗的临床疗效显著,可减少溶栓药物总用量,缩短溶栓及住院时间,减少出血风险,值得临床推广。
Objective To study the effect of Angiojet mechanical thrombus aspiration combined with catheter-directed thrimbolysis(CDT)on lower extremity deep venous thrombosis(LEDVT).Method 60 LEDVT patients were selected from the First Affiliated Hospital of Jiamusi University from July 2017 to April 2019.According to the different ways of thrombolysis,the patients were divided into control group and observation group,30 cases each.The total effective rate and treatment of the two groups were compared.At 6 months after operation,the patency of the vein and the occurrence of post thrombotic syndrome(PTS)were evaluated.Result The total effective rate of observation group was 100.00%higher than that of control group 73.33%;the dosage of urokinase was lower than that of control group(P<0.05).After 6 months,the patency rate of the observation group was 86.67%,which was higher than that of the control group 33.33%(P<0.05).The PTS rate of the observation group was 3.33%lower than that of the control group 13.33%,the difference was not statistically significant.Conclusion Angiojet mechanical thrombus aspiration combined with CDT is effective in the treatment of LEDVT,which can reduce the total dosage of thrombolytic drugs,shorten the time of thrombolysis and hospitalization,and reduce the risk of bleeding,which is worth popularizing.
