腹腔镜下子宫血管阻断治疗子宫肌瘤的临床疗效

Clinical effect of laparoscopic uterine vascular occlusion on uterine leiomyoma

目的探讨腹腔镜下子宫血管阻断治疗子宫肌瘤的临床疗效。方法选取中国人民解放军第一七四医院于2017年1月至2018年12月行腹腔镜子宫肌瘤剔除术的子宫肌瘤患者85例,根据手术方式不同分为观察组40例患者接受腹腔镜下子宫血管阻断术联合子宫肌瘤剔除术,对照组45例患者接受腹腔镜子宫肌瘤剔除术。统计并比较两组患者手术情况、术后并发症发生率和术后恢复时间;术前、术后第2天性激素指标、血常规指标和免疫功能指标。结果两组患者手术用时比较,差异无统计学意义;观察组患者术中出血量低于对照组患者,术后并发症发生率2.50%低于对照组患者15.56%,术后恢复时间低于对照组患者;术后第二天,观察组患者性激素指标、血常规指标和免疫功能指标均高于对照组患者,差异有统计学意义(P<0.05)。结论腹腔镜下子宫血管阻断术可减少子宫肌瘤患者术中出血量,减轻手术操作对患者性激素、血常规、免疫功能的影响,有利于减少术后并发症,加快术后康复。

Objective To explore the clinical effect of laparoscopic uterine vascular occlusion in the treatment of hysteromyoma.Method From January 2017 to December 2018,85 patients with hysteromyoma underwent laparoscopic myomectomy in the 174 th Hospital of the people’s Liberation Army.According to the different operation methods,they were divided into observation group(40 patients)who underwent laparoscopic uterine vascular occlusion combined with hysteromyomectomy,and control group(45 patients)who underwent laparoscopic hysteromyomectomy.Statistics and comparison of the two groups of patients with surgery,postoperative complications and postoperative recovery time;preoperative and postoperative second nature hormone indicators,blood routine indicators and immune function indicators.Result There was no significant difference between the two groups in the time of operation.The blood output in the observation group was lower than that in the control group,the incidence of postoperative complications was 2.50%lower than that in the control group 15.56%,and the recovery time was lower than that in the control group.On the second day after operation,the indexes of sex stimulating hormone,blood routine test and immune function in the observation group were higher than those in the control group,with statistical difference significance(P<0.05).Conclusion Laparoscopic uterine vascular blockade can reduce the amount of bleeding in the operation of hysteromyoma patients,reduce the impact of the operation on the sexual hormone,blood routine,immune function,and help to reduce postoperative complications and accelerate postoperative rehabilitation.
