酒石酸布托啡诺联合舒芬太尼对腹腔镜子宫肌瘤切除术后患者疼痛及血流动力学的影响分析

Effect of Butorphanol Tartrate Combined with Sufentanil on Pain and Hemodynamics in Patients after Laparoscopic Myomectomy

目的探究腹腔镜子宫肌瘤切除术后患者采用酒石酸布托啡诺联合舒芬太尼对其疼痛、血流动力学的影响。方法选取2021年8月至2022年8月在我院收治的80例腹腔镜子宫肌瘤切除术患者,按随机数字表法分为对照组和观察组各40例。对照组采用舒芬太尼,观察组采用酒石酸布托啡诺联合舒芬太尼。比较两组拔管后10 min(T1)、拔管后20 min(T2)、拔管后30min(T3)的血流动力学[平均动脉压(MAP)、心率(HR)]、疼痛程度[视觉模拟评分法(VAS)]、镇静程度(Ramsay镇静评分),并对比两组不良反应发生率。结果两组T1、T2、T3间的MAP、HR水平比较,差异无统计学意义(P>0.05)。两组T2、T3的VAS评分较T1时低,且T3时VAS评分较T2低,观察组各时点的VAS评分较对照组低,差异有统计学意义(P<0.05)。两组T2、T3的Ramsay镇静评分较T1时高,且T3时Ramsay镇静评分较T2时高,且观察组各时点的Ramsay镇静评分较对照组高,差异有统计学意义(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论酒石酸布托啡诺联合舒芬太尼对腹腔镜子宫肌瘤切除术后患者疼痛、躁动有较好的缓解效果,且不会影响患者血流动力学,不增加不良反应。

Objectives To investigate the effects of butorphanol tartrate combined with sufentanil on pain and hemodynamics in patients after laparoscopic myomectomy.Methods A total of 80 patients who underwent laparoscopic myomectomy admitted to our hospital from August 2021 to August 2022 were selected and randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with sufentanil,and the observation group was treated with butorphanol tartrate combined with sufentanil.The hemodynamics[mean arterial pressure(MAP),heart rate(HR)],pain degree[visual analogue scale(VAS)],and sedation degree(Ramsay sedation score)of the two groups were compared at 10 min(T1),20 min(T2),and 30 min(T3)after extubation,and the incidence of adverse reactions were compared between the two groups.Results There was no significant difference in MAP and HR levels between the two groups at T1,T2 and T3(P>0.05).The VAS score of the two groups at T2 and T3 were significantly lower than those at T1,and the VAS score at T3 was significantly lower than that at T2.The VAS score of the observation group at each time point were significantly lower than those of the control group(P<0.05).The Ramsay sedation scores at T2 and T3 were significantly higher than those at T1,the Ramsay sedation scores at T3 were significantly higher than those at T2,and the Ramsay sedation scores of observation group at each time point were significantly higher than those of the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusions Butorphanol tartrate combined with sufentanil achieves a good effect in relieving pain and restlessness in patients after laparoscopic myomectomy,without affecting hemodynamics and increasing adverse reactions.
