楔形照射和平野照射技术在全脑三维适形放疗中的剂量学研究

摘要 目的研究肺癌脑转移放疗计划过程中采用三维适形(3D-CRT)楔形照射和平野照射技术对全脑PTV等剂量包裹体积、适形性及均匀性指数的影响。方法随机选取治疗的肺癌原发灶经病理学证实的脑转移患者56例,对患者的定位影像分别采用三维适形楔形照射和平野照射技术制定全脑放疗计划,对两种放射治疗技术计划的剂量学指标进行有效观察和比较。结果56例患者两种计划比较发现,采用楔形照射技术的放疗计划剂量覆盖度和均匀性与采用平野照射技术组比较相对理想,但靶区适形性指数无明显优势。结论两种三维照射技术均能满足靶区的覆盖和剂量学要求,但楔形照射技术较平野照射技术95%靶区处方剂量和剂量均匀性具有一定优势,能给予全脑靶区更好的剂量学要求。
