磁敏感加权成像(SWI)在高血压患者脑微出血的价值研究 被引量:2

摘要 目的:探讨研究磁敏感加权成像(SWI)在高血压患者脑微出血的价值。方法:选取2017年6月-2018年12月对120例高血压患者进行进行常规T1WI、T2WI、T2-FLAIR序列和DWI及SWI序列检查。观察比较不同的MRI序列在脑微出血方面的检测率及显示情况,观察不同高血压分级患者的脑微出血分级情况。结果:SWI序列检测CMBs的检出例数为59例,DWI序列检测CMBs的检出例数为45例,T2FLAIR序列检测CMBs的检出例数为35例,T2WI序列检测CMBs的检出例数为30例,T1WI序列检测CMBs的检出例数为28例,SWI序列在脑微出血方面的检出例数显著高于其他序列,(P<0.05)。SWI序列在脑微出血方面的(检出率)检查数目方面显著高于其他检查序列(Z=6.190,P<0.05)。在SWI序列所检查出的172个病灶当中,位置最多的为基底节区:65个(37.8%)其次是丘脑:47个(27.3%),皮层及皮层下:40个(23.3%),脑干:12个(7.0%),小脑:8个(4.7%)。脑微出血分级跟高血压分级之间存在相关性(r=0.316,P<0.05),主要表现为高血压分级不断升高,脑微出血分级越高。结论:给予高血压脑微出血患者使用磁敏感加权成像(SWI)检查,可更加清晰、更为明显的检出脑微出血病灶,为临床诊疗提高更加准确的参考信息。 Objective:to study the value of magnetic sensitivity weighted imaging(SWI)in cerebral microhemorrhage in patients with hypertension.Methods:120 patients with hypertension were selected from June 2017 to December 2018 for routine T1 WI,T2 WI,t2-flair sequence and DWI and SWI sequence examination.The detection rate and display of cerebral microhemorrhage in different MRI sequences were observed and compared,and the grading of cerebral microhemorrhage in patients with different hypertension grades were observed.Results:the detection of cases CMBs SWI sequence detection of 59 cases,DWI sequence can detect CMBs detection and 45 cases of T2 FLAIR sequence detection detection cases of 35 cases of CMBs,T2 WI sequence detection detection cases of 30 cases of CMBs,T1 WI sequential detection detection of cases to 28 cases of CMBs,SWI sequence detection in micro cerebral hemorrhage were significantly higher than that of other sequences,(P<0.05).SWI sequence was significantly higher than other sequences in the number of examinations(detection rate)for cerebral microhemorrhage(Z=6.190,P<0.05).Of the 172 lesions detected by SWI sequence,65(37.8%)were located in basal ganglia region,followed by 47(27.3%)in thalamus,40(23.3%)in cortex and subcortex,12(7.0%)in brain stem,and 8(4.7%)in cerebellum.There was a correlation between the grade of cerebral microhemorrhage and the grade of hypertension(r=0.316,P<0.05).The main manifestation was that the grade of hypertension was increasing,and the grade of cerebral microhemorrhage was higher.Conclusion:magnetic sensitive weighted imaging(SWI)examination for patients with hypertensive intracerebral microhemorrhage can more clearly and significantly detect the intracerebral microhemorrhage,which can provide more accurate reference information for clinical diagnosis and treatment.
