经会阴三维、四维超声在产后前盆腔功能的超声评估

Evaluation of Three-dimensional and Four-dimensional Ultrasonography of Perineum on the Function of Antepartum Pelvis

目的:探讨经会阴三维、四维超声在产后前盆腔功能中的应用价值。方法:选取2019年6月-2020年1月期间我院收治的患有前盆腔功能障碍的患者29例纳入观察组,选取盆腔功能正常的25例纳入对照组,两组疾病的诊断依据均为产后42天常规筛查结果。利用经会阴三维、四维超声检查方法对比两组盆腔脱垂情况及在静止期和张力期前盆腔膀胱移动度、膀胱后角开放、膀胱膨出、尿道漏斗形成、裂孔扩张的情况。结果:经对比后发现,观察组的盆腔功能障碍显著高于对照组,差异具有统计学意义(P<0.05)。Valsalva状态下,观察组在张力期及静止期所对应的BSD、膀胱移动度、膀胱后角开放和尿道旋转角度显著高于对照组,差异具有统计学意义(P<0.05)。静息状态下,两组比较无统计学意义(P>0.05)。结论:经会阴三维、四维超声的应用给临床医生提供了更详细的疾病资料,让患者得到了及时有效的治疗,在临床上有显著地应用价值,值得推广。

Objective:To explore the application value of three-dimensional and four-dimensional transperineal ultrasound in the function of the antepartum pelvis.Methods:from June 2019 to January 2020,29 patients with anterior pelvic dysfunction admitted to our hospital were included in the observation group,and 25 patients with normal pelvic function were included in the control group.The diagnosis of the two groups was based on the results of routine 42 day postpartum screening.The three-dimensional and four-dimensional ultrasonography were used to compare the two groups’pelvic prolapse,the mobility of the bladder,the opening of the posterior horn of the bladder,the slight bulging of the bladder,the formation of the urethra funnel,the prolapse of the uterus,the rectocele and the dilatation of the hiatus.Results:after comparison,the distance of pelvic prolapse in the observation group was significantly higher than that in the control group(P<0.05).Under the condition of Valsalva,the BSD,bladder mobility,posterior angle opening and urethra rotation angle of the observation group were significantly higher than those of the control group(P<0.05).There was no significant difference between the two groups in resting state(P>0.05).Conclusion:the application of three-dimensional and four-dimensional transperineal ultrasound provides clinicians with more detailed disease information,and enables patients to get timely and effective treatment,which has significant clinical application value and is worth promoting.
