超声造影对冠心病患者颈动脉斑块内新生血管的诊断价值

Diagnostic Value of Contrast-enhanced Ultrasound in Carotid Plaque in Patients with Coronary Heart Disease

目的:探讨超声造影(CEUS)对冠心病(CHD)患者颈动脉斑块内新生血管的诊断效能。方法:回顾性分析2016年1月~2019年1月间收治的108例颈动脉斑块患者临床资料,根据冠脉造影结果分为非CHD组(n=28)、CHD单支病变组(n=48)、CHD多支病变组(n=32)。所有患者均行CEUS,评估各组患者颈动脉斑块情况、不同类型CHD患者颈动脉斑块类型,采用超声造影时间-强度曲线定量指标评估斑块内新生血管。结果:①CHD多支病变组软斑块占比高于CHD单支病变组和非CHD组(P<0.05),CHD单支病变组软斑块占比高于非CHD组(P<0.05);②急性冠脉综合征患者软斑块占比高于稳定性CHD患者(P<0.05);③CHD多支病变组达峰时间低于CHD单支病变组和非CHD组(P<0.05),CHD单支病变组达峰时间低于非CHD组(P<0.05),CHD多支病变组最大峰值强度比、增强密度均高于CHD单支病变组和非CHD组(P<0.05),CHD单支病变组最大峰值强度比、增强密度均高于非CHD组(P<0.05)。结论:CEUS能评估CHD患者颈动脉斑块及斑块内新生血管状况,有利于临床CHD诊治。

Objective:To evaluate the diagnostic efficacy of CEUS in the diagnosis of carotid artery plaque in patients with coronary heart disease(CHD).Methods:The clinical data of 108 patients with carotid plaque from January 2016 to January 2019 were analyzed retrospectively. The results of coronary angiography were divided into non-CHD group(n=28), CHD single-branch lesion group(n=48), and multiple coronary artery disease group(n=32). All patients underwent CEUS to evaluate the carotid plaque of each group. assessing neovascularization within the plaque.Results:The proportion of soft spots in CHD patients was higher than that in CHD single branch disease group and non-CHD group(P<0.05), the proportion of soft spot in CHD single branch disease group was higher than that in non-CHD group(P<0.05). The degree of intensity was higher than that of CHD single vessel lesion group and non-CHD group(P<0.05). The maximum peak intensity ratio and enhanced density of, CHD single vessel lesion group were higher than those of non CHD group(P<0.05).Conclusion:CEUS can evaluate carotid plaques and neovascularization in patients with CHD, which is beneficial to the diagnosis and treatment of CHD.
