骨性Ⅱ类错[牙合]畸形青少年Twin-blcok治疗前后舌位及上气道变化

Changes of tongue position and upper airway before and after Twin-block treatment in skeletal class Ⅱ malocclusion children

目的探索骨性Ⅱ类青少年患者Twin-block治疗后舌体积、位置变化,以及与上气道变化的相关性。方法Twin-block配合上颌慢速扩弓治疗下颌后缩的骨性Ⅱ类患者24例为实验组,进行“2*4”固定矫治的骨性Ⅰ类患者24例为对照组,使用Mimics 21.0软件测量治疗前后舌体积、位置变化,应用Dolphin 11.8软件测量治疗前后上气道的变化。数据使用SPSS 22.0统计软件进行统计学分析。结果治疗前后舌体垂直位置的增加量,实验组大于对照组(P<0.05);实验组治疗后与治疗前相比,舌体积(TV)、舌体水平位置显著增加(P<0.05);治疗前,实验组V_(舌)、S_(舌)、V_(喉)、S_(喉)小于对照组(P<0.05),治疗后无统计学差异。结论骨性Ⅱ类青少年Twin-block矫治后,舌体垂直位置降低,需进行舌位置及舌功能训练,防止骨性Ⅱ类复发导致上气道阻塞。

Objective This study is to explore changes in tongue volume,position,and correlation with changes in upper airway after Twin-block treatment in skeletal classⅡchildren patients.Methods Twenty-four cases of skeletal classⅡpatients with mandibular retrusion were treated with Twin-block and maxillary slow expansion,and 24 cases of skeletal classⅠpatients with"2*4"fixed treatment were treated as control group.The changes of tongue volume and position before and after treatment were measured by Mimics 21.0 software,and the changes of upper airway before and after treatment were measured by Dolphin 11.8 software.Data were analyzed by SPSS22.0 statistical software.Results The increase of vertical position of tongue before and after treatment in the experimental group was greater than that in the control group(P<0.05).Compared with pretreatment,TV and the horizontal position of tongue in experimental group increased significantly(P<0.05).Before treatment,V_(舌)、S_(舌)、V_(喉)、S_(喉)of the experimental group were smaller than those of the control group(P<0.05),and there was no statistical difference after treatment.Conclusion The tongue vertical position of skeletal classⅡchildren after Twin-block treatment is reduced,so training of tongue position and tongue function is needed to prevent the upper airway obstruction caused by the recurrence of skeletal classⅡ.
