摘要
目的探究超声检查在血液透析患者心脏病变诊断中的应用效果。方法选取2021年1月-2023年12月我院收治的167例患者,87例血液透析患者作为观察组,80例同时间段于我院进行健康体检人群作为对照组,所有患者接受多普勒超声心动图检查,对比两组患者右心室功能、分析右室射血分数与四腔心、两腔心切面瓣环连线中点处的收缩期峰值位移(FTADmid、TTADmid)以及其占百分比(FTADmid%、TTADmid%)之间的相关性。结果观察组RVEDV、RVESV、RVSV、RVCO高于对照组,RVEF低于对照组(P<0.05);观察组FTADmid、TTADmid、FTADmid%、TTADmid%低于对照组(P<0.05);血液透析患者RVEF与FTADmid、FTADmid%、TTADmid、TTADmid%具有显著相关性,相关系数分别为r=0.95、0.88、0.92、0.89(P<0.01)。结论通过超声检查的二维斑点追踪技术可以准确评估患者右心室功能的变化,治疗后心脏负荷增加与RVEF具有显著相关性,可以用于评估血液透析患者心脏病变。
Objective:To explore the effect of ultrasound examination in the diagnosis of cardiac abnormalities on hemodialysis patients.Methods:A total of 167 patients admitted to our hospital from January 2021 to December 2023 were selected,87 hemodialysis patients as the observation group,and 80 healthy people who underwent physical examination in our hospital during the same period as the control group.All patients received Doppler echocardiography.Right ventricular function[right ventricular end-diastolic volume(RVEDV),right ventricular stroke volume(RVSV),right ventricular end-systolic volume(RVESV),right ventricular output per minute(RVCO),and right ventricular ejection fraction(RVEF)]were compared between the two groups.The correlation between right ventricular ejection fraction and peak systolic displacement(FTADmid,TTADmid)and its percentage(FTADmid%,TTADmid%)at the midpoint of the four-chamber heart and two-chamber heart incision plane flap ring connection was analyzed.Results:The RVEDV,RVESV,RVSV and RVCO of the observation group were higher than those of the control group,and the RVEF of the observation group was lower than that of the control group(P<0.05).FTADmid,TTADmid,FTADmid%and TTADmid%in the observation group were lower than those in the control group(P<0.05);In hemodialysis patients,RVEF was significantly correlated with FTADmid,FTADmid%,TTADmid%,and TTADmid%,and the correlation coefficients were r=0.95,0.88,0.92 and 0.89(P<0.01).Conclusion:Two-dimensional speckle tracking technology through ultrasound examination can detect changes in right ventricular function in hemodialysis patients.The significant correlation between increased cardiac load after treatment and RVEF can be used to assess cardiac abnormalities in hemodialysis patients.
