全膝关节置换术后患者CRP、ESR、患膝皮温的变化规律及相互关系

Changes of CRP,ESR and Skin Temperature of the Affected Knee after Total Knee Arthroplasty and Their Relationship

摘要 目的研究全膝关节置换术后患者C反应蛋白(CRP)、红细胞沉降率(ESP)与患膝皮温变化规律及相关性。方法选取2018年1月-2023年6月在我院接受全膝关节置换术的患者90例,于术前,术后1 d、3 d、7 d,术后4周、12周患侧膝关节皮温、健侧膝关节皮温,双侧皮温差,分析双侧皮温差与CRP、ESR变化规律及相关性。结果术前,双侧膝关节皮温对比无显著差异(P>0.05);术后1 d、3 d、7 d、术后4周患膝皮温显著高于健膝(P<0.05)。患者双侧皮温差、CPR及ESP均呈现升高后下降趋势,双侧皮温差术后3d达高峰,术后12周恢复正常,CPR术后3 d达高峰,术后12周恢复正常,ESP术后7 d达高峰,术后12周恢复正常,Pearson相关性分析显示,双侧皮温差与CPR呈正相关,与ESP呈正相关。结论全膝关节置换术患者受到非感染性炎症反应影响,导致CPR、ESP升高,患膝关节皮肤温度增加。 Objective:To study the correlation between C-reactive protein(CRP),erythrocyte sedimentation rate(ESP)and skin temperature of affected knee in patients after total knee arthroplasty.Methods:A total of 90 patients who underwent total knee arthroplasty in our hospital from January 2018 to June 2023 were selected.The skin temperature of affected knee joint,skin temperature of healthy knee joint and bilateral skin temperature difference before surgery,on 1 d,3 d,and 7 d after surgery,at 4 weeks after surgery and 12 weeks after surgery were analyzed for their changes and correlation with CRP and ESR.Results:There was no significant difference in skin temperature of bilateral knee joints before surgery(P>0.05).The skin temperatures of the affected knees were significantly higher than those of the healthy knees on 1 d,3 d,7 d,and at 4 weeks after surgery(P<0.05).The bilateral skin temperature difference,CPR and ESP all showed a trend of decreasing after increasing.The bilateral skin temperature difference reached the peak on 3 d after surgery and returned to normal at 12 weeks after surgery.It reached the peak on 3 d after surgery with CPR and returned to normal at 12 weeks after surgery.It reached the peak on 7 d after surgery with ESP and returned to normal at 12 weeks after surgery.Pearson correlation analysis showed that the bilateral skin temperature difference was positively correlated with CPR and ESP.Conclusion:Patients undergoing total knee arthroplasty were affected by non-infectious inflammatory reaction,which lead to elevated CPR and ESP as well as increased skin temperature of the affected knee.
