连续性血液滤过在慢性肾衰竭合并脓毒血症患者中的早期应用效果

Early Effect of Continuous Hemofiltration in Patients with Chronic Renal Failure Complicated with Sepsis

摘要 目的分析连续性血液滤过在慢性肾衰竭合并脓毒血症患者中的早期应用效果。方法2020年1月-2023年12月本院收治的慢性肾衰竭合并脓毒血症患者68例,将其平均分为对照组和实验组,每组各34例。对照组患者接受临床常规治疗,在此基础上给予实验组患者连续性血液滤过治疗。对比分析两组患者治疗前后炎症因子、生化指标、血流动力学指标之间的差异。结果治疗前,炎症因子、生化指标、血流动力学指标水平组间比较,无显著差异(P>0.05)。治疗后,两组患者炎症指标、生化指标以及血流动力学指标均得到显著改善,具体表现为:炎症因子、生化指标水平均降低,血流动力学指标水平提高,且实验组改善程度明显优于对照组,组内、组间比较均差异显著(P<0.05)。结论针对慢性肾衰竭合并脓毒血症患者,在临床常规治疗基础上联合连续性血液滤过治疗,可获得理想的炎症控制效果、稳定的血流动力学效果和生化指标改善效果,临床推广应用价值较高。 Objective:To analyze the early effect of continuous hemofiltration in patients with chronic renal failure complicated with sepsis.Methods:From January 2020 to December 2023,68 patients with chronic renal failure complicated with sepsis admitted to our hospital were selected,and were equally divided into the control group and the experimental group,with 34 cases in each group.Patients in the control group received clinical routine treatment,and patients in the experimental group were given continuous hemofiltration therapy on this basis.The differences of inflammatory factors,biochemical indicators,and hemodynamic indicators before and after treatment were compared and analyzed between the two groups.Results:Before treatment,there was no significant difference in the levels of inflammatory factors,biochemical indexes,and hemodynamic indexes between groups(P>0.05).After treatment,the inflammatory indicators,biochemical indicators,and hemodynamic indicators of patients in both groups were significantly improved.The specific manifestations are as follows:the levels of inflammatory factors and biochemical indicators decreased,and the level of hemodynamic indicators increased.The improvement degree of the experimental group was significantly better than that of the control group,with significant differences within and between groups(P<0.05).Conclusion:For patients with chronic renal failure complicated with sepsis,continuous hemofiltration based on clinical routine treatment can obtain ideal inflammation control,stable hemodynamic effect,and biochemical index improvement,which is of high value for clinical promotion and application.
