手术室配合和护理对人工全膝关节置换术患者的预后影响

The Effect of Operating Room Cooperation and Nursing on Prognosis of Patients Undergoing Artificial Total Knee Replacement

目的探讨人工全膝关节置换术手术护理中采取手术室配合和护理对预后的影响。方法选取2018年1月至2021年6月我院收治的122例人工全膝关节置换术患者,依据双盲法分组,每组各61例。对照组采取传统护理,观察组采取手术室配合和护理干预。比对不同时段视觉模拟量表(VAS)、日常生活能力(Katz指数)、膝关节评分(HSS)、膝关节屈曲度≥90°的时间、住院时间、切口愈合程度、并发症、恢复优良率等指标。结果观察组术后6、24 h VAS评分较对照组低(p<0.05);观察组Katz指数术前比对照组无差异(p>0.05),观察组Katz指数术后比对照组高(p<0.05);观察组术前HSS评分比对照组无差异(p>0.05),观察组术后2周、1个月、3个月、6个月HSS评分比对照组高(p<0.05);观察组膝关节屈曲度≥90°的时间、住院时间比对照组短(p<0.05);观察组切口愈合程度、恢复优良率比对照组优(p<0.05);观察组深静脉血栓、感染、水肿发生概率低于对照组(p<0.05)。结论在人工全膝关节置换术手术室护理中做好手术操作配合工作,优化护理计划,HSS评分升高,住院时间缩短,疼痛程度减轻,并发症发生风险降低,临床上可大力推广应用。

Objective:To discuss the effect of operating room cooperation and nursing on prognosis of patients undergoing artificial total knee replacement.Methods:A total of 122 patiehts with artificial total knee replacement in our hospital from January 2018 to June 2021 were selected,and were divided according to the double-blind method,with 61 cases in each group.The control group received traditional nursing,and the observation group received operating room cooperation and nursing intervention.The visual simulation scale(VAS),daily living ability(Katz index),knee joint score(HSS),time of knee flexion 90,time of hospital stay,incision healing degree,complications,recovery rate were compared.Results:Lower VAS score was observed at 6 and 24 h than that of in the control group(p<0.05);There was no difference between Katz indexes than that of control group(p>0.05).The Katz index in the observation group was higher than that in the control group(p<0.05);There was no difference in the preoperative HSS score compared with the control group(p>0.05).The HSS score of the observation group was higher than that of the control group at 2 weeks,1,3,and 6 months after operation(p<0.05);The time and hospital stay of knee flexion≥90 in the observation group were shorter than those in the control group(p<0.05);The degree of incision healing and recovery rate of the observation group were better than the control group(p<0.05);The probability of deep vein thrombosis,infection and edema was lower than that of the control group(p<0.05).Conclusion:In the nursing of the operating room for artificial total knee replacement,it is important to cooperate with the surgical procedures,optimize the nursing plan,increase the HSS score,shorten the hospital stay,reduce pain levels,and effectively avoid complications,which can be promoted in clinic.
