纳布啡联合舒芬太尼用于老年单侧全髋关节置换术后静脉自控镇痛的效果

Effect of Nalbuphine Combined with Sufentanil on Controlled Intravenous Analgesia after Unilateral Total Hip Arthroplasty in Elderly Patients

摘要 目的观察纳布啡联合舒芬太尼用于老年单侧全髋关节置换术后静脉自控镇痛的效果。方法选取2020年1月-2022年1月在我院接受单侧全髋关节置换术治疗的老年患者60例,随机分为对照组和实验组,每组各30例。术后给予对照组患者舒芬太尼静脉自控镇痛,实验组患者联合纳布啡进行静脉自控镇痛。对比分析两组患者术后镇痛效果(视觉模拟评分法,visual analogue scale,VAS)、镇静效果(Ramsay镇静评分,ramsay sedation score,RSS)以及苏醒期治质量。结果手术完成后,VAS、RSS评分组间比较,差异无统计学意义(P>0.05);术后2、12、24 h,实验组VAS、RSS评分均明显低于对照组,差异显著(P<0.05)。结论老年单侧全髋关节置换术后静脉自控镇痛应用纳布啡联合舒芬太尼可获得理想的镇痛、镇静效果,并且可以缩短患者苏醒时间以及降低躁动程度。 Objective:To observe the effect of nalbuphine combined with sufentanil on controlled intravenous analgesia after unilateral total hip arthroplasty in elderly patients.Methods:A total of 60 elderly patients who underwent unilateral total hip arthroplasty in our hospital from January 2020 to January 2022 were selected,and were randomly divided into control group and experimental group,with 30 cases in each group.Patients in the control group were given sufentanil and patients in the experimental group were given nalbuphine combined with intravenous controlled analgesia.The postoperative analgesic effect(visual analogue scale,VAS),sedation effect(Ramsay sedation score,RSS)and recovery treatment quality were compared between the two groups.Results:After operation,there was no significant difference in VAS and RSS scores between groups(P>0.05).The VAS and RSS scores of the experimental group were significantly lower than those of the control group at 2,12 and 24 h after operation,with significant differences(P<0.05).Conclusion:The application of nalbuphine combined with sufentanil for controlled intravenous analgesia after unilateral total hip arthroplasty in elderly patients can achieve ideal analgesic and sedative effects,shorten the recovery time and reduce the degree of agitation.
