生物型与骨水泥型髋关节置换术治疗股骨颈骨折患者的临床疗效

Clinical Effect of Biological and Cement Hip Arthroplasty in the Treatment of Patients with Femoral Neck Fracture

摘要 目的探讨生物型与骨水泥型髋关节置换术治疗股骨颈骨折患者的临床效果。方法选取2019年1月-2021年12月在我院接受髋关节置换术治疗的64例股骨颈骨折患者,根据随机数字表法将其分为对照组和研究组,每组各32例。对照组使用骨水泥型假体,研究组使用生物型假体,观察两组患者手术时间、术中出血量、术后引流量,术后并发症发生情况,入组时、术后12月髋关节功能恢复情况。结果研究组手术时间短于对照组(P<0.05)。研究组术中出血量、术后引流量少于对照组(P<0.05)。研究组术后并发症发生率为3.13%低于对照组的25.00%。术后12月,两组患者中文版牛津髋关节功能量表(Oxford hip score,OHS)各条目评分及总评分均有下降,研究组总评分为(14.52±2.75)分低于对照组(16.37±3.19)分(P<0.05)。结论生物型人工假体手术时间短、术后骨水泥相关并发症发生风险较低,远期髋关节功能恢复情况较好,对于无严重骨质疏松的高龄股骨颈骨折患者生物型人工假体为首选。 Objective:To study the clinical effect of biological and cement hip arthroplasty in the treatment of patients with femoral neck fracture.Methods:A total of 64 patients with femoral neck fracture who underwent hip arthroplasty in our hospital from January 2019 to December 2021 were selected,and were divided into control group and study group according to the random number table method,with 32 cases in each group.The control group was treated with cement prosthesis,and the study group was treated with biological prosthesis.The operation time,intraoperative bleeding,postoperative drainage volume,postoperative complications,hip function recovery at the time of enrollment and 12 months after operation were observed.Results:The operation time in the study group was shorter than that in the control group(P<0.05).The amount of intraoperative bleeding and postoperative drainage in the study group were less than those in the control group(P<0.05).The incidence of postoperative complications in the study group was 3.13%,lower than 25.00%in the control group.The scores of each item and the total score of the Chinese version of Oxford hip score(OHS)in the two groups decreased at 12 months after operation.The total score of the study group was(14.52±2.75)lower than that of the control group(16.37±3.19)(P<0.05).Conclusion:Biological prosthesis has the advantages of short operation time,low risk of postoperative bone cement related complications,and good long-term recovery of hip function.Biological prosthesis can be the first choice for elderly patients with femoral neck fracture without serious osteoporosis.
