ICU综合护理干预股骨头置换术患者对生活质量和髋关节功能的影响

Effect of ICU Comprehensive Nursing on Quality of Life and Hip Function in Patients Undergoing Femoral Head Replacement

目的分析股骨头置换术患者在围术期采取ICU综合护理对髋关节功能和生活质量的影响。方法选取2019年1月-2021年12月我院收治的80例股骨头置换术患者,采用双盲法将其分为对照组和观察组,每组各40例。对照组采取传统护理计划,观察组采取ICU综合护理计划,对比近期效果指标、髋关节功能水平(Harris髋关节功能量表评估髋关节功能水平)、生活质量分值(SF-36量表评估)、并发症情况等研究数据。结果观察组近期效果指标(首次离床时间、12 h疼痛强度、住院时间)显著低于对照组,72 h睡眠时间长于对照组(p<0.05);观察组髋关节功能指标(活动范围、髋关节疼痛、下肢畸形、功能状态)较对照组高(p<0.05);观察组SF-36量表中各维度分值显著高于对照组(p<0.05);观察组并发症(假体脱位、便秘、肺部感染)发生率相比于对照组低(p<0.05)。结论股骨头置换术后患者通过ICU综合护理指导后,近期各指标水平得到明显改善,髋关节功能有所提升,生活水平也随之升高,各种并发症指标得到良好控制,值得临床推广应用。

Objective:To analyze the effect of ICU comprehensive nursing on hip function and quality of life in patients with femoral head replacement can take various nursing measures during the perioperative period.Methods:From January 2019 to December 2021,80 patients with femoral head replacement were selected,and were divided by double-blind,with 40 cases in each group.The control group adopted the traditional nursing,and the observation group adopted the ICU comprehensive nursing to compare the recent effect indicators,hip function level(hip function level assessed by the Harris hip function scale),quality of life score(SF-36 scale assessment),and complications.Results:The recent effect index(first departure time,12 h pain intensity,hospital time)in the observation group were significantly lower than the control group(p<0.05),72 h sleep time was longer than the control group;in the observation group hip function index(range of activity,hip pain,lower limb deformity,functional status)were higher than the control group(p<0.05);the SF-36 scale was significantly higher than the control group(p<0.05);the probability of complications(prosthetic dislocation,constipation,lung infection)was lower than control group(p<0.05).Conclusion:After the guidance of ICU,various indicators have been significantly improved,hip function has been improved,the quality of life has increased,and various complications have been well controlled,which can be vigorously promoted and applied in clinical practice.
