八邪操联合经络按摩在维持性血液透析患者AVF促成熟中的应用

Application of Baxie Exercise Combined with Meridian Massage in Promoting Maturation of AVF in Patients with Maintenance Hemodialysis

目的观察八邪操联合经络按摩在维持性血液透析患者动静脉内瘘(AVF)促成熟中的应用。方法选取2019年1月至2021年10月于我院接受动静脉内瘘成型术的80例终末期肾病维持性血液透析患者,收治于奇偶数病室的病例各为40例,分为对照组与试验组,对照组按动静脉内瘘成型术护理常规施护,试验组在此基础上加用八邪操与经络按摩,对两组术后4周、8周、内瘘首次穿刺血流量,内瘘穿刺时间及成功率,由专业的医生对并发症严重程度进行评分,包括切口渗血与肢体肿胀、血管栓塞与吻合口狭窄,对两组动静脉内瘘成型术者治疗前后进行疼痛度测评。结果试验组在首次穿刺、术后4周、8周血流量均高于对照组(P<0.05);试验组的首次穿刺时间和穿刺成功率均高于对照组(P<0.05);试验组患者并发症切口渗血、肢体肿胀、血管栓塞与吻合口狭窄严重程度评分均低于对照组(P<0.05);治疗前两组患者疼痛评分无明显差异(P>0.05),治疗后两组患者疼痛评分均下降,且试验组低于对照组(P<0.05)。结论采用八邪操联合经络按摩对维持性血液透析患者开展护理,可以显著提升内瘘使用时的血流量,降低术后疼痛感受与相关并发症发生率,促内瘘成熟作用良好。

Objective:To observe the application of Baxie exercise combined with meridian massage in promoting the maturation of arteriovenous fistula(AVF)in patients with maintenance hemodialysis.Methods:From January 2019 to October 2021,80 patients with end-stage renal disease undergoing maintenance hemodialysis in our hospital were selected as the study sample,and 40 patients were admitted to odd and even rooms respectively,which were divided into control group and experimental group.The control group was given routine nursing according to arteriovenous fistula surgery.On this basis,the experimental group was given Baxie exercise combined with meridian massage for postoperative fistula puncture in the first time within 4 weeks,8 weeks,blood flow,the time of internal fistula puncture and the success rate of the two groups were scored by professional doctor to the of severity of complication,including incision oozing with limb swelling,vascular embolization,and anastomotic stricture,the pain degree of arteriovenous fistula was evaluated before and after treatment.Results:The blood flow of the experimental group was higher than that of the control group at first puncture,4 weeks and 8 weeks after operation(P<0.05).The first puncture time and puncture success rate of experimental group were higher than those of control group(P<0.05).The score of severity of complications of incision bleeding,limb swelling,vascular embolism and anastomotic stenosis in experimental group was lower than that in control group(P<0.05).Before treatment,there was no significant difference in pain degree between two groups(P>0.05);but after treatment,pain degree in two groups decreased,and the experimental group was lower than the control group(P<0.05).Conclusion:The combination of Baxie exercise and meridian massage can significantly improve the blood flow during the use of internal fistula,reduce postoperative pain and the incidence of related complications,and promote the maturation of internal fistula.
