认知行为护理结合饮食护理对糖尿病肾病血液透析患者营养状况及肾功能的影响

Effect of Cognitive Behavioral Nursing Combined with Diet Care on Nutritional Status and Renal Function of Hemodialysis Patients with Diabetic Nephropathy

目的探讨认知行为护理结合饮食护理对糖尿病肾病血液透析患者营养状况及肾功能的影响。方法选取2018年1月至2020年6月我院收治的糖尿病肾病血液透析患者72例,按随机数字表法将其分为两组,每组各36例。对照组给予常规护理,观察组实施认知行为护理结合饮食护理。比较两组营养状况及肾功能。结果观察组转铁蛋白(TRF)、白蛋白(AIB)、前白蛋白(PA)、每日所摄入蛋白含量(DPI)及蛋白分解率(n PCR)均高于对照组,血尿素氮(BUN)、肌酐清除率(Ccr)高于对照组,血肌酐(Scr)低于对照组,差异有统计学意义(P<0.05)。结论认知行为护理结合饮食护理能够调节糖尿病肾病血液透析患者饮食结构,改善营养状况及肾功能,具有推广价值。

Objective This paper aims to explore the effect of cognitive behavioral nursing combined with diet care on the nutritional status and renal function of hemodialysis patients with diabetic nephropathy.Methods A total of 72 hemodialysis patients with diabetic nephropathy admitted to our hospital from January 2018 to June 2020 were selected and divided into two groups according to the random number table method,with 36 cases in each group.The control group was given routine nursing,and the observation group was given cognitive behavioral nursing combined with diet care.The nutritional status and renal function of the two groups were compared.Results The transferrin(TRF),albumin(AIB),prealbumin(PA),daily protein intake(DPI)and protein decomposition rate(n PCR)of the observation group were all higher than those of the control group.The blood urea nitrogen(BUN)and creatinine clearance(Ccr)were higher than the control group,and serum creatinine(Scr)was lower than the control group,the difference was statistically significant(P<0.05).Conclusion Cognitive behavioral nursing combined with diet care can adjust the dietary structure of hemodialysis patients with diabetic nephropathy,improve nutritional status and renal function,which has promotion values.
