超声引导下神经阻滞复合喉罩全麻在人工全髋置换手术中的应用

Application of Ultrasound-guided Nerve Block Combined with Laryngeal Mask General Anesthesia in Total Hip Arthroplasty

目的探讨超声引导下神经阻滞复合喉罩全麻在人工全髋置换手术(THA)中的应用效果。方法选取2019年4月-2021年4月于我院行THA治疗的68例患者,按随机数字表法将其分为常规组及超声引导组,每组各34例。常规组采用静脉复合全麻气管插管,超声引导组采用超声引导下神经阻滞复合喉罩全麻。比较两组苏醒时间、视觉模拟评分法(VAS)评分及不良反应。结果超声引导组苏醒时间短于常规组,术后2、12 h时VAS评分低于常规组,不良反应总发生率低于常规组,有统计学差异(P<0.05)。结论THA术中采用超声引导下神经阻滞复合喉罩全麻具有术后镇痛效果好、苏醒快速及不良反应少等优点,用于THA麻醉中安全有效。

Objective This paper aims to investigate the application effect of ultrasound-guided nerve block combined with laryngeal mask anesthesia in total hip arthroplasty(THA).Methods 68 patients who underwent THA treatment in our hospital from April 2019 to April 2021 were selected and divided into conventional group and ultrasound-guided group by the random number table method,with 34 cases in each group.The conventional group was used for endotracheal intubation combined with venous anesthesia,and the ultrasound-guided group was used for ultrasound-guided nerve block combined with laryngeal mask anesthesia.Resuscitation time,visual analogue scale(VAS)score and adverse reactions were compared between the two groups.Results The resuscitation time of the ultrasound-guided group was shorter than that of the conventional group,the VAS score at2 h and 12 h after surgery was lower than that of the conventional group,and the total incidence of adverse reactions was lower than that of the conventional group,with statistical differences(P<0.05).Conclusion Ultrasound-guided nerve block combined with laryngeal mask general anesthesia in THA has the advantages of good postoperative analgesia,rapid recovery and less adverse reactions,which is safe and effective for THA anesthesia.
