不同入路方式行人工肱骨头置换治疗肱骨近端粉碎性骨折疗效对比

Comparative Study on the Effect of Different Approaches of Artificial Humeral Head Replacement in the Treatment of Comminuted Proximal Humeral Fractures

摘要 目的研究不同入路方式行人工肱骨头置换治疗肱骨近端粉碎性骨折疗效。方法选取上饶市妇幼保健院骨科于2018年1月至2020年2月收治的肱骨近端粉碎性骨折患者109例,采用随机数字表法将其分为对照组和治疗组。结果治疗组围术期指标及术后并发症发生情况均低于对照组;术后肩关节功能Neer评分优良率高于对照组(P<0.05)。结论经三角肌外侧入路方式能有效减少人工肱骨头置换术手术时间,降低患者术中出血量,使术后并发症发生率大大降低治疗效果优于三角肌胸大肌间隙入路方式。 Objective This paper aims to study the effect of different approaches of artificial humeral head replacement in the treatment of comminuted proximal humeral fractures.Methods 109 patients with comminuted fractures of the proximal humerus were selected from the Department of Orthopedics of Shangrao Maternal and Child Health Hospital from January 2018 to February 2020,which were randomly divided into control group and treatment group by number table method.Results Perioperative indicators and postoperative complications in the treatment group were lower than those in the control group;after surgery,the excellent and good rate of postoperative shoulder joint function Neer score is higher than the control group(P<0.05).Conclusion The lateral approach through the deltoid muscle can effectively reduce the surgical time of artificial humeral head replacement,thereby reducing the amount of blood loss during the operation,decreasing the incidence of postoperative complications,the treatment effect is better than the approach of the deltoid pectoralis major muscle space.
