左卡尼汀联合血液透析治疗尿毒症性周围神经病变的临床观察

Clinical Effect of Levocarnitine Combined with Hemodialysis in the Treatment of Uremia Peripheral Neuropathy

目的对比研究单纯血液透析(HD)治疗尿毒症性周围神经病变(UPN)与HD+左卡尼汀联合治疗的疗效差异,对联合治疗效果进行判定,从而选出UPN最有效性的治疗方法。方法2018年全年收治的UPN患者中抽选120例进行研究,利用抛色子分组法将其分成联合组(60例,HD+左卡尼汀治疗)与单一组(60例,HD治疗),分别比较治疗效果、不良症状发生率及神经传递速度。结果联合组治疗有效率为96.7%,高于单一组的81.7%,且治疗后各症状发生率仅为13.3%,低于单一组的35.0%,差异均有统计学意义(P<0.05)。联合组神经传递速度改善情况比单一组更好,差异均有统计学意义(P<0.05)。结论HD治疗尿毒症时会引发UPN,而在应用HD治疗时联合左卡尼汀,能够起到较好的治疗效果,临床治疗有效率较高,不良症状的发生情况较少,对神经传递速度的改善也优于单纯HD治疗,因此,联合治疗效果全面优于单一治疗,可作为优质方案进行推广。

Objective This paper aims to compare the curative effect of hemodialysis(HD)and HD combined with Lcarnitine in the treatment of uremic peripheral neuropathy(UPN),and to determine the combined treatment effect,so as to select the most effective method for UPN patients.Methods 120 cases of UPN patients admitted in the whole year of 2018 were selected,who were divided into combination group(60 cases,HD combined with L-carnitine treatment)and single group(60 cases,HD treatment).The effective rate,incidence of adverse symptoms and nerve transmission velocity were compared.Results The effective rate of the combined group was 96.7%,significantly higher than 81.7%of the single group,P<0.05.The incidence of adverse symptoms in the combined group was 13.3%,significantly lower than 35.0%of the single group,P<0.05.The improvement of nerve transmission velocity in the combined group was significantly better than that of the single group,P<0.05.Conclusion HD treatment of uremia will cause UPN,and the application of HD combined with L-carnitine can play a better therapeutic effect.It can obtain higher clinical treatment effective rate and lower incidence of adverse symptoms,and the improvement of nerve transmission speed is better than HD treatment.Therefore,the combined treatment effect is better than single treatment,which could be popularized as a high-quality program.
