经尿道膀胱肿瘤电切术在膀胱癌合并维持性血液透析患者中的应用效果

Application Effect of Transurethral Resection of Bladder Tumor in Patients with Bladder Cancer Combined with Maintenance Hemodialysis

目的探究经尿道膀胱肿瘤电切术在膀胱癌合并维持性血液透析患者中的应用效果。方法回顾性分析2010年1月-2016年12月我院收治膀胱癌合并维持性血液透析患者60例,分为研究者和对照组,两组病例各30例。所有患者均行维持性血液透析治疗,对照组行膀胱部分切除术,研究组行经尿道膀胱肿瘤电切术。对比两组围术期指标、并发症发生率、生存率及复发率。结果研究组手术时间、留置导尿管时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组术后平均生存时间长于对照组,差异有统计学意义(P<0.05);两组复发率及生存率比较,差异无统计学意义(P>0.05)。结论经尿道膀胱肿瘤电切术在膀胱癌合并维持性血液透析患者治疗中有助于延长患者生存时间,减少术中出血量,降低并发症发生率,安全性高。

Objective This paper aims to explore the application effect of transurethral resection of bladder tumor in patients with bladder cancer combined with maintenance hemodialysis.Methods 60 patients with bladder cancer and maintenance hemodialysis were divided into two groups,30 cases in each group.All patients were treated with maintenance hemodialysis,the control group was treated with partial cystectomy,and the study group was treated with transurethral resection of bladder tumor.The perioperative indexes,complication rate,survival rate and recurrence rate were compared between the two groups.Results The operation time,indwelling catheter time and hospitalization time of the study group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,the incidence of complications was lower than that of the control group,and the average survival time after operation was longer than that of the control group(P<0.05);There was no significant difference in recurrence rate and survival rate between the two groups(P>0.05).Conclusion Transurethral resection of bladder tumor in the treatment of bladder cancer patients with maintenance hemodialysis is helpful to prolong the survival time of patients,decrease the amount of bleeding,and reduce the incidence of complications,which has high safety.
