心脏直视手术中心肌缺血性挛缩(石样心综合征)—附5例报告

Myocardial Ischemic Contracture(Stone Heart Syndrome)in Open-Heart-Surgery:Report of 5 Cases

心肌缺血性挛缩(下称石样心)是心脏直视手术的少见并发症,预后极差。1972年Cooley首次报告在4732例心脏手术中发生石样心13例,无一例存活。我们在临床曾遇到典型石样心5例,现报告并分析如下。

Ischemic contracture of the heart(stone heart)is a serious complication of open heart surgery.We encountered 5 such cases among 4632 open heart operations.Stone heart syndrome developed at the end of the operation.Severe left ventricular dysfunction appeared immediately during the removal of the cardiopulmonary bypass and was characterized by that the left ventricle remained firmly and stone-like contracted.Despite of the resumption of cardiopulmonary bypass,using of pressor agents and propranolol,cardiac pacing,and intraaortic balloons,etc.intractable ventracular fibrillation and hypoperfusion led to the patients death.This report emphasized that the prevention of its occurrence is of prime importance in dealing with this kind of serious complication and improved measures for the protection of myocardium are suggested.
