延续护理对鼻咽癌放疗患者自我管理效能感影响的Meta分析

Meta-analysis of the effects of continuous nursing on self-management efficacy of nasopharyngeal carcinoma patients with radiotherapy

目的:系统评价延续护理对鼻咽癌放疗患者自我管理效能感的影响。方法:检索多个中英文论文数据库,收集鼻咽癌放疗患者有关延续护理随机对照试验,利用RevMan5.3软件行Meta分析。结果:共纳入5篇随机对照研究,共612例患者。Meta分析结果显示,延续护理干预3个月可改善鼻咽癌放疗患者自我管理效能感的正性态度[MD=4.59,95%CI(2.70,6.47),Z=4.77,P<0.00001]、自我决策[MD=2.38,95%CI(1.93,2.82),Z=10.41,P<0.00001]、自我减压[MD=4.69,95%CI(3.37,6.01),Z=6.96,P<0.00001]维度;而延续护理干预6个月可改善鼻咽癌放疗患者自我管理效能感的正性态度[MD=4.12,95%CI(2.53,5.71),Z=5.08,P<0.00001]、自我决策[MD=3.06,95%CI(2.80,3.32),Z=22.88,P<0.00001]维度。结论:延续护理干预可提高鼻咽癌放疗患者自我管理效能感。

Objective:To systematically evaluate the effects of continuous nursing on self-management efficacy of nasopharyngeal carcinoma(NPC)patients with radiotherapy.Method:Multiple Chinese and English paper databases were searched to collect randomized controlled trials of continuous nursing for NPC patients with radiotherapy.Meta-analysis was performed using RevMan 5.3 software.Results:Five randomized controlled studies were included,involving 612 patients.Meta-analysis showed that continuous nursing intervention for 3 months improved the positive attitude dimension[MD=4.59,95%CI(2.70,6.47),Z=4.77,P<0.00001],self-decision-making dimension[MD=2.38,95%CI(1.93,2.82),Z=10.41,P<0.00001]and selfdecompression dimension[MD=4.69,95%CI(3.37,6.01),Z=6.96,P<0.00001]of self-management efficacy of nasopharyngeal carcinoma radiotherapy patients,while continuing nursing intervention for 6 months improved the positive attitude dimension[MD=4.12,95%CI(2.53,5.71),Z=5.08,P<0.00001]and self-decision dimension[MD=3.06,95%CI(2.80,3.32),Z=22.88,P<0.00001]of self-management efficacy of nasopharyngeal carcinoma radiotherapy patients.Conclusion:Continuous nursing intervention can improve the self-management efficacy of NPC patients with radiotherapy.
