难治性胃巨大溃疡合并大出血抢救成功1例的诊治体会及文献复习

Diagnosis and treatment of sudden massive hemorrhage in patient with intractable giant gastric ulcer: case report and literature review

目的:提高对胃巨大溃疡所致上消化道大出血的诊疗认识。方法:分析我科胃巨大溃疡患者突发大出血的临床资料1例,并结合文献进行讨论。结果:该患者突发大出血后一度生命垂危,后经内外科积极配合,抢救获得成功。结论:对胃巨大溃疡出血患者要严密监测生命体征,并应及时行急诊胃镜检查,明确溃疡情况;经常规内科保守治疗,但仍有呕血或便血者应及时行内镜下治疗或进行外科干预。

Objective:To improve the diagnosis and treatment of massive upper gastrointestinal hemorrhage caused by giant gastric ulcer.Methods:The clinical data of a patient with giant gastric ulcer suffering from sudden massive hemorrhage in our department was analyzed and discussed in combination with the references.Results:The patient’s life was in danger for a time after sudden massive hemorrhage,and was rescued successfully together with internal medicine and surgery.Conclusion:Vital signs of patients with massive gastric ulcer bleeding should be closely monitored,and timely emergency gastroscopy should be performed to determine the ulcer situation;after regular internal medicine conservative treatment,hematemesis or hematochezia still being observed,timely endoscopic treatment or surgical intervention should be adopted.
