健脑颗粒对后循环缺血患者的疗效及患者颈动脉斑块、脑血流的变化研究 被引量:6

摘要 目的:分析健脑颗粒对后循环缺血患者的疗效及患者颈动脉斑块、脑血流的变化。方法:选取我院于2017年5月—2018年8月收治的104例后循环缺血患者的临床资料进行分析,分为2组。对照组采用常规治疗,观察组在常规治疗的基础上给予健脑颗粒治疗,分析2组患者治疗后的临床疗效。结果:2组患者治疗前颈动脉斑块组间比较,差异不具有统计学意义(P>0.05)。观察组患者治疗后斑块大小、斑块厚度、血管中膜厚度低于对照组(P<0.05)。2组患者治疗前内皮素-1(ET-1)、降钙素基因相关肽(CGRP)、血栓调节蛋白(TM)、血管性血友病因子(vWF)、血小板衍生生长因子-BB(PDGF-BB)、血栓素B2(TXB2)、血小板α颗粒蛋白-140(GMP-140)、溶血磷脂酸(LPA)水平组间比较,差异不具有统计学意义(P>0.05)。观察组患者治疗后ET-1、TM、vWF、PDGF-BB、TXB2、GMP-140、LPA水平低于对照组,CGRP水平高于对照组(P<0.05)。2组患者治疗前双侧椎动脉、基底动脉的脑血流动力学水平组间比较,差异不具有统计学意义(P>0.05)。观察组患者治疗后双侧椎动脉、基底动脉的峰值血流速度(Vs)、平均血流速度(Vm)水平高于对照组,搏动指数(PI)水平低于对照组(P<0.05)。结论:健脑颗粒治疗后循环缺血,可降低患者斑块厚度,改善患者脑血流水平。 Objective:to analyze the effect of Jiannao granule on patients with posterior circulation ischemia and the changes of cerebral blood flow in patients with carotid artery plaque.Methods:The clinical data of 104 patients with posterior circulation ischemia admitted to our hospital from May 2017 to August 2018 were selected for analysis.The patients were divided into control group,who received conventional treatment,and observation group,who were treated with Jiannao granule on top of conventional treatment.The clinical efficacy of the two groups was analyzed after treatment.Results:The difference of carotid plaque between the two groups was not statistically significant(P>0.05).The size of the plaque,the thickness of the plaque and the media thickness of the media in the treated group were lower than that of the control group(P>0.05).The levels of ET-1,TM,vWF,PDGF-BB,TXB2,GMP-140 and LPA in the observation group were lower than that of the control group(P>0.05).The peak blood flow velocity(Vs),mean blood flow velocity(Vm)and mean blood flow velocity(Vm)of the bilateral vertebral artery and basilar artery after treatment in the observation group were higher than that of the control group(P<0.05).Conclusion:After the treatment of cerebral ischemia,jiannao granule can reduce the thickness of plaque and improve the level of cerebral blood flow.
