项目导向培训考核对肿瘤化疗患者中心静脉导管并发症的影响

Effect of Project-oriented Training Assessment on Central Venous Catheter Complications in Oncology Chemotherapy Patients

目的:探讨项目导向培训考核对肿瘤化疗患者中心静脉导管并发症的影响。方法:对护士实施中心静脉导管维护项目为导向的培训考核,收集2019年3月至2019年12月福建省肿瘤医院内科病房中心静脉置管744例次,分析导管并发症及留置时间,评价培训效果。结果:本研究中36例(4.8%)发生非计划性拔管,其中33例为导管滑脱,3例为导管相关性血流感染,无发生中心静脉导管堵管、导管内血栓及导管断裂等情况。未发生导管相关并发症708例次(95.2%),平均导管留置时间为(19.18±8.5)d,时间跨度为1~193 d。结论:项目导向培训考核可提升临床导管维护及宣教的有效率,降低导管并发症的发生率,从而延长导管留置时间,保证护理顺利进行。

Objective To investigate the impact of project-oriented training assessment on central venous catheter complications in oncology chemotherapy patients.Methods The nurses were trained and assessed on the implementation of the central venous catheter maintenance project.A total of 744 cases of central venous catheterization in the internal medicine ward of Fujian Cancer Hospital from March 2019 to December 2019 were collected,and the catheter complications and indwelling time were analyzed to evaluate the training effect.Results In this study,unplanned extubation occurred in 36 cases(4.8%),including 33 cases of catheter slippage and 3 cases of catheter-related bloodstream infection.No central venous catheter occlusion,thrombosis or catheter fracture occurred.No catheter-related complications occurred in 708 cases(95.2%).The average catheter indwelling time was(19.18±8.5)d,and the time span was 1-193 days.Conclusion Project-oriented training and assessment can improve the efficiency of clinical catheter maintenance and education,reduce the incidence of catheter complications,so as to prolong the catheter indwelling time and ensure the smooth progress of nursing.
