成人健康体检者血清脂蛋白相关磷脂酶A2活性水平及影响因素分析

Level of serum lipoprotein associated phospholipase A2 activity and its influencing factors in adult health examinees

目的调查湖南省某医院成人健康体检者血清脂蛋白相关磷脂酶A2(lipoprotein associated phospholipase A2,Lp-PLA2)的活性水平,探讨年龄、性别及生活方式等因素的影响。方法随机选择408例体检者,采用连续监测法测定血清Lp-PLA2活性。所有数据采用SPSS 22.0软件进行统计分析。结果Lp-PLA2活性水平在血脂指标中与三酰甘油、胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、载脂蛋白B的相关系数(95%CI)分别为0.233(0.496~0.628)、0.483(0.405~0.554)、0.483(0.404~0.554)、-0.194(-0.285~0.099)、0.566(0.496~0.628)。回归分析显示Lp-PLA2活性水平与性别(β=0.227,P<0.001)和年龄(β=0.247,P<0.001)有相关性。男性Lp-PLA2活性水平(487.0±147.0)U/L高于女性(422.0±128.0)U/L,差异有统计学意义(t=4.707,P<0.001)。Lp-PLA2活性水平在各年龄段间的差异有统计学意义(F=6.813,P<0.001),随着年龄增加有升高趋势。根据性别进行分析时,男性各年龄段的Lp-PLA2活性水平差异无统计学意义(F=1.693,P=0.152),女性各年龄段的Lp-PLA2活性水平差异有统计学意义(F=9.164,P<0.001)。进一步分析显示40岁以上女性的Lp-PLA2活性水平高于40岁以下女性,差异有统计学意义(t=-6.339,P<0.001)。结论本研究表明Lp-PLA2活性水平与性别和年龄独立相关,与饮酒、吸烟情况、身体质量指数、血压状况无相关性,在血脂指标中与载脂蛋白B关系最密切。

Objective To investigate the level of serum lipoprotein associated phospholipase A2(Lp-PLA2)activity in adult health examinees from a hospital in Hunan province,and to explore the influence of factors such as age,gender and lifestyle.Methods Four hundred and eight health examinees were randomly selected,and continuous monitoring assay was used to detect the level of serum Lp-PLA2 activity.All the data were statistically analyzed using SPSS 22.0 software.Results The correlation coefficients(95%CI)of Lp-PLA2 with triglyceride,cholesterol,low density lipoprotein-cholesterol,high density lipoprotein-cholesterol,and apolipoprotein A1 in blood lipid indexes were 0.233(0.496-0.628),0.483(0.405-0.554),0.483(0.404-0.554),-0.194(-0.285-0.099),and 0.566(0.496-0.628),respectively.Regression analysis showed a correlation of Lp-PLA2 activity with gender(β=0.227,P<0.001)and age(β=0.247,P<0.001).The level of serum Lp-PLA2 activity was higher in the male group than in the female group((487.0±147.0)U/L vs.(422.0±128.0)U/L),with a statistically significant difference(t=4.707,P<0.001).The level of serum Lp-PLA2 activity showed statistically significant differences among different age groups(F=6.813,P<0.001),and there was an increasing trend with age.No statistically significant difference was found in the level of Lp-PLA2 activity among men from different age groups(F=1.693,P=0.152),but statistically significant differences were found in the level of Lp-PLA2 activity among women from different age groups(F=9.164,P<0.001).Further analysis showed that the level of Lp-PLA2 activity was higher in women over 40 years of age than in women under 40 years of age,showing a statistically significant difference(t=-6.339,P<0.001).Conclusions This study shows that the level of Lp-PLA2 activity is independently related to gender and age,but has no correlation with drinking,smoking,body mass index and blood pressure status.It is most closely related to ApoB in blood lipid indicators.
