双止血带法外周静脉留置术应用于宫外孕失血性休克患者的效果

Effect of double Tourniquet method for peripheral venous indwelling in patients with hemorrhagic shock of ectopic pregnancy

目的探讨双止血带法外周静脉留置术应用于宫外孕失血性休克患者的效果。方法选择2020年6月至2021年11月本院收治的98例宫外孕失血性休克患者作为研究对象,按照随机数字表法分为对照组和观察组,各49例。对照组患者使用单根止血带法外周静脉留置术治疗,观察组患者使用双止血带法外周静脉留置术治疗。对比两组患者的穿刺效果、围术期情况、穿刺相关并发症发生情况、休克相关并发症与死亡发生情况。结果观察组的一次穿刺成功率为95.92%,高于对照组的83.67%(χ^(2)=4.009,P=0.045)。两组的平均失血量与止血时间对比差异无统计学意义(P>0.05)。观察组的穿刺相关并发症发生率为12.24%,对照组为8.16%,对比差异无统计学意义(χ^(2)=0.446,P=0.505)。观察组的休克相关并发症发生率为4.08%,低于对照组的16.32%(χ^(2)=4.009,P=0.045),两组均无死亡患者。结论相较于单根止血带法外周静脉留置术,双止血带法可提高宫外孕失血性休克患者的一次穿刺成功率,减少休克相关并发症的发生,且不会增加患者的失血量和穿刺相关并发症风险,亦不会延长止血时间。

Objective To explore the effect of double Tourniquet in peripheral vein indwelling in patients with hemorrhagic shock of ectopic pregnancy.Methods 98 patients with ectopic pregnancy hemorrhagic shock admitted to our hospital from June 2020 to November 2021 were selected as the study subjects.They were randomly divided into a control group and an observation group,with 49 patients in each group,using a random number table method.The patients in the control group were treated with a single Tourniquet and the patients in the observation group were treated with a double Tourniquet.Puncture effect,perioperative situation,occurrence of puncture related complications,shock related complications,and mortality were observed between the two groups.Results The success rate of one puncture in the observation group was 95.92%,higher than 83.67%in the control group(χ^(2)=4.009,P=0.045).There was no statistically significant difference in the average blood loss and hemostasis time between the two groups(P>0.05).The incidence of puncture related complications was 12.24%in the observation group and 8.16%in the control group,with no statistically significant difference(χ^(2)=0.446,P=0.505).The incidence of shock related complications in the observation group was 4.08%,lower than 16.32%in the control group(χ^(2)=4.009,P=0.045),and there were no deaths in either group.Conclusions Compared with the single Tourniquet method for peripheral vein indwelling,the double Tourniquet method can improve the one-time puncture success rate of patients with hemorrhagic shock of ectopic pregnancy,reduce the incidence of shock related complications,and will not increase the blood loss of patients and the risk of puncture related complications,nor extend the hemostasis time.
