心房颤动患者华法林抗凝疗效的危险因素分析

Analysis of Risk Factors of Anticoagulant Effect of Warfarin in Patients with Atrial Fibrillation

目的分析心房颤动(以下简称房颤)患者华法林抗凝疗效的危险因素。方法选取2016年1月—2019年1月三台县人民医院收治的房颤患者82例,根据不同给药方法分为对照组和观察组,各41例。两组患者均给予常规治疗,对照组患者给予阿司匹林治疗,观察组患者予以华法林治疗。比较两组患者的临床疗效及维持窦性心率发生率,采用单因素及多因素Logistic回归分析探讨房颤患者华法林抗凝疗效的影响因素。结果两组患者复律发生率比较,差异无统计学意义(P>0.05);观察组患者维持窦性心律发生率高于对照组(P<0.05)。单因素分析结果显示:脑卒中病史、年龄、合并用药可能是房颤患者华法林抗凝疗效的影响因素(P<0.05)。多因素Logistic回归分析结果显示:脑卒中病史、年龄<60岁是房颤患者华法林抗凝疗效的危险因素(P<0.05)。结论华法林抗凝治疗可有效维持房颤患者窦性心律,且脑卒中病史、年龄<60岁是影响房颤患者华法林抗凝疗效的危险因素。

Objective To investigate the risk factors of anticoagulant effect of warfarin in patients with atrial fibrillation.Methods 82 cases of atrial fibrillation were selected from January 2016 to January 2019 in Santai County People’s Hospital,and they were divided into control group and observation group according to the different medications,41 cases in each group.All patients were given basic treatment,patients in control group were given aspirin,patients in observation group were given warfarin.The clinical effect and sinus rhythm were compared between the two groups.Risk factors of anticoagulant effect of warfarin in patients with atrial fibrillation were analysed by univariate and multivariate Logistic regression analysis.Results No significant difference of incidence of compound law between the two groups(P>0.05);incidence of maintain sinus rhythm in the observation group was significantly higher than that in the control group(P<0.05).Univariate regression analysis showed that,history of stroke,age,combination drugs may were influencing factors of anticoagulant effect of warfarin in patients with atrial fibrillation(P<0.05).Multivariate Logistic regression analysis showed that,history of stroke,age<60 years old were risk factors of anticoagulant effect of warfarin in patients with atrial fibrillation(P<0.05).Conclusion Warfarin can effectively maintain sinus rhythm of patients with atrial fibrillation,and history of stroke,age<60 years old are risk factors of anticoagulant effect of warfarin in patients with atrial fibrillation.
