新产程标准对产程中剖宫产指征的变化及母婴结局的影响

The Influence of New Stage of Labor Standard on the Changes of Cesarean Section Indications and Maternal and Infant Outcomes

目的探讨新产程标准对产程中剖宫产指征的变化和母婴结局的临床应用影响。方法以2016年1月—2019年1月在江苏省常州市中医医院接受治疗的160例接受剖宫产产妇作为研究对象,按照时间不同进行分类:对照组(n=80,2016年1月—2017年1月就诊患者采用原产程标准),观察组(n=80,2018年1月—2019年1月就诊患者采用新产程标准),比较两组最终的剖宫产指征和母婴结局。结果观察组患者剖宫产胎儿窘迫、产程异常、社会因素以及转剖宫产较对照组显著减少;前置胎盘和疤痕子宫较对照组显著增加,且观察组发生产后出血率11.25%和产后尿潴留率13.75%都显著高于对照组(P<0.05)。结论新产程标准能显著降低产程中转剖宫产,改善剖宫产各指征的变化,但容易出现产后出血、产后尿潴留、前置胎盘和疤痕子宫,需做好相关预防工作。

Objective To explore the influence of the new stage of labor standard on the changes of cesarean section indications and the clinical application of maternal and infant outcomes.Methods 160 cases of cesarean section women who received treatment in Changzhou Hospital of Traditional Chinese Medicine from January 2016 to January 2019 were selected as the research objects,and they were classified according to different time:the control group(n=80,the patients in the treatment group from January 2016 to January 2017 adopted the original process standard),and the observation group(n=80,the new labor process standard was adopted for the patients who visited the hospital from January 2018 to January 2019).Results The observation group had significantly lower fetal distress,labor abnormalities,social factors and cesarean section than the control group.The placenta previa and scar uterus increased significantly compared with the control group,and the incidence of bleeding after delivery and the retention rate of urine in the observation group were 13.75%and 11.25%,respectively,which were significantly higher than those in the control group(P<0.05).Conclusion The new labor standard can significantly reduce the cesarean section during labor,improve the indications of cesarean section,but it is easy to have postpartum hemorrhage,postpartum urinary retention,placenta previa and scar uterus.
