超声引导下PTCD治疗治疗恶性梗阻性黄疸的临床疗效及安全性分析

Effect of Left Hepatic Pedicle Occlusion with Left Liver minimally Invasive Surgery on the Clinical Efficacy and Complications of Patients with Intrahepatic Cholelithiasis

目的探究左肝蒂阻断行左半肝微创手术对肝内胆管结石患者的临床疗效及患者并发症发生的影响。方法选取2017年4月—2019年4月时间段内来重庆西南铝医院行手术治疗的肝内胆管结石的患者98例,随机将其分为研究组与对照组,各49例,作为此次研究的对象。对照组采用常规解剖性左半肝切除手段进行临床治疗,研究组采用左肝蒂阻断行左半肝微创手术进行临床治疗,观察两组患者术前、术后肝功能指标的水平变化情况以及并发症的发生率。结果研究组治疗后ALT、AST、TBIL、ALB的水平变化均低于对照组(P<0.05),研究组并发症发生率低于对照组(P<0.05)。结论与临床常规性治疗手段相比,左肝蒂阻断行左半肝微创手术对于肝内胆管结石患者的治疗效果显著,其操作简便、造成的创面较小、康复速度快等优点值得在临床中广泛应用以及研究。

Objective To explore the clinical effect of left hepatic pedicle occlusion for minimally invasive surgery of left liver on patients with intrahepatic bile duct stones and the effect of complications.Methods 98 cases of patients with intrahepatic bile duct stones who came to Chongqing Southwest Alu minum Hospital for surgical treatment from April 2017 to April 2019 were selected and randomly divided into two groups,namely the study group and the control group,with 49 cases as the subjects of this study.The control group received conventional anatomic left liver resection for clinical treatment,and the research group received minimally invasive left liver surgery with left liver pedicle occlusion for clinical treatment.The changes of liver function indexes before and after surgery and the incidence of complications in the two groups were observed.Results After treatment,ALT,AST,TBIL and ALB levels in the study group were all lower than those in the control group(P<0.05),and the incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion Compared with conventional clinical treatment,minimally invasive left liver surgery with left hepatic pedicle occlusion has a significant therapeutic effect on patients with intrahepatic bile duct stones,and its advantages of simple operation,small wound and quick recovery are worthy of extensive application and research in clinical practice.
